A Defined Algorithm for Regenerative Success Part 1

Description:
In the rapidly changing world of bone regeneration, biotechnology has taken the forefront. Bioengineering, bioactive materials, recombinant and autologous growth factors have changed the landscape of wound healing and reduced the morbidity of our regenerative procedures. Additionally, technology related to surgical armamentarium has also changed with the advent of Piezoelectric and micro surgical tools aimed at reduced trauma and efficiency. Grafting materials, membrane technologies, tenting screws, ti-mesh etc. have also played an integral role in the changing face of regenerative therapies. These presentations will focus open the above concepts but prioritize the diagnostic phase and surgical common denominators required for a successful regenerative outcome. Flap design, space maintenance, bone and membrane selections and tension free closure provides the template for all the new age materials and technologies to be successful in clinical practice.

In this lecture and video based Case Report, a patient with multiple recession defects in the Maxillary Anterior Region who was first treated with Tunnel Approach and ACDM grafts reveals breakdown of the result at 3 years in the area of the Maxillary Left Central Incisor with further recession and erosion of the root. The Semi-Lunar Approach is highlighted as a minimally invasive microsurgical approach to this situation. Incision design, instrumentation and suturing are highlighted. Post-op results are displayed.

Soft tissue recession around teeth and implants is an epidemic problem in the field of dentistry. A predictable and long-term soft tissue augmentation outcome is primarily dependent on proper diagnosis, prognosis and treatment planning. Esthetic Correction of Gingival Recession Using a Modified Tunnel Technique and an Acellular Dermal Connective Tissue Allograft
Esthetic correction of gingival recession is an important goal of periodontal therapy. This article describes a surgical technique that combines a modified tunnel technique and an acellular dermal connective tissue allograft. With the aid of vertical incisions, a tunnel is created under the buccal mucosa of the affected tooth. These incisions enable easy access for graft placement and create mobility for gingival coronal positioning. The use of an accelular dermal connective tissue allograft eliminates the need for a surgical palatal donor site. This minimizes post-surgical complications.
