A Defined Algorithm for Regenerative Success Part 1

Description:
In the rapidly changing world of bone regeneration, biotechnology has taken the forefront. Bioengineering, bioactive materials, recombinant and autologous growth factors have changed the landscape of wound healing and reduced the morbidity of our regenerative procedures. Additionally, technology related to surgical armamentarium has also changed with the advent of Piezoelectric and micro surgical tools aimed at reduced trauma and efficiency. Grafting materials, membrane technologies, tenting screws, ti-mesh etc. have also played an integral role in the changing face of regenerative therapies. These presentations will focus open the above concepts but prioritize the diagnostic phase and surgical common denominators required for a successful regenerative outcome. Flap design, space maintenance, bone and membrane selections and tension free closure provides the template for all the new age materials and technologies to be successful in clinical practice.

The "Modified Tunnel" Technique and the development of the Lateral Tunnel Approach
The "Modified Tunnel" technique is a method of esthetic connective tissue grafting that protects the interdental papillae. First published in 1999, this lateral tunnel approach uses vertical incisions on both sides of the treatment area. These vertical incisions facilitate placement and positioning of the graft over the root surfaces without the need to disturb the interdental papillae. This technique is very useful in esthetic areas and those that involve several teeth. An acellular dermal connective tissue allograft is an excellent choice of graft material to use with this technique.

Immediate placement and loading protocols are the most technique sensitive and at the highest risk of complication, especially in the aesthetic zone. The peri-implant tissues undergo a resorption pattern that may see exposure of parts supporting the restoration, otherwise intended to be submerged, with extreme aesthetic compromise or even complete failure of treatment. The literature is not definitive in terms of any one treatment modality to recover such a complication.
