Summary:

Alternatives to restore defective and deficient edentulous spans should include prosthetic gingival restoration as an integral part of an overall esthetic reconstructive option in the decision-making process.To reestablish natural crown ratios and natural gingival profiles in complex cases, artificial gingiva restorations can reduce the necessity of technique-sensitive surgical procedures that depend on the individual pattern of biologic repair, thereby simplifying and reducing the time and cost of treatment.

Predictable and repeatable results can be achieved through materials and clinical practices that, on one hand, preserve the original structure of the compromised tooth and, on the other hand, involve a very low biologic price. New technologies for the enamel-dentin adhesion and the continuous evolution of more sophisticated esthetic materials (composite resins and porcelains) allow the effective reaching of verifiable and predictable results. This lecture aims to point out the relevant guidelines for obtaining predictable indirect posterior restorations through the evaluation of a correct biomechanical approach rather than giving just a “trendy” adhesive approach.
