3D Reconstruction of an Atrophic Ridge

Description:
Following extraction of a compromised central incisor the remaining site is not amendable to implant placement. This “over the shoulder� procedure details the reconstruction utilizing an Allograft J-Block shaped to fit the defect on a stereolitho model derived from an I-Cat Scan and Materialise Software. The use of Plasma Rich Growth Factor (PRGF), and the unique fibrin membrane is similarly textured. The appropriate use of both mattress suture and interrupted ties to maintain soft tissue closure as well as post-op healing is critical to success.

Omid; They all work as you said. It really depends on preference and experience of the surgeon. As we continue to attempt to be LESS INVASIVE in our REGENERATIVE approaches, I seem to favor less Autologous blocks if possible and use more GBR and bioactive modifiers like PRGF, PRF and BMP-2. For these procedures where there is much bone to regenerate I prefer non-resorbable barriers. regards Dr. Salama

Rodolfo and Nicolas; Thank you. Yes, the allograft block is prepared on the sterile stereolithographic model the same day as the patient surgery and we hydrate the block in saline and then PRGF for an additional 10 minutes. As for the MGJ, yes that must be managed by additional CTG at time of Bone Block or at 2nd Stage with apical positioning of the outer flap.

Amazing procedure and totally agree with everything. Initially right after the surgery I can see a displacement of the MGJ towards the crest and afterwards 11 days post op I can see the repositioning of the MGJ apically. Is that achieved thanks to the CTG that you placed under the flap? Thank you very much Dr Salama

Dr. Durrani; Perhaps the most important aspect of any regenerative procedure is stability of the graft and tension free closure. We achieved both on this case. It turned out very well and has been loaded in function for almost 2 years. I will post these images in the near future. thanks again for your interest

Fibrin-Guided Bone Regeneration: Guidelines of a Simple, Predictable, & Low Risk Technique
This lecture is focused on an effective regenerative approach, but at low knowledge. The goal is to provide a new skill in favor of well-established techniques. The Fibrin Sealant (FS) is an additional clot of human origin. It has adhesive, sealant and inductive action stimulating hard and soft-tissue regeneration. The FS, mixed with bone chips and biomaterials, allows a better handling of the graft that, becoming plastic and moldable, adapts perfectly to bony defects without dispersion of granules. It’s effective also on wound protection achieving a faster healing of soft-tissue. This approach is excellent in the socket preservation and in the peri-implant regeneration on "closed" sites where the implants are inside the defects.
