Management of Narrow and Short Ridge Dimensions in Implant Dentistry: New Minimally Invasive Concepts
The management of narrow and short ridge dimensions has posed a great concern for most implantologists. Often large GBR or block augmentation procedures are suggested with high morbidity but this lecture presents more minimally invasive options to restore these ridge challenges. Ridge Expansion with motorized drills as well as the utilization of ultra-short and wider implants to avoid sinus and nerve proximity challenges will be discussed. The role of blood born bioactive modifiers, specifically PRGF (Plasma Rich in Growth Factors) to enhance wound healing will similarly be highlighted.

The Full Arch Patient, A Fully Digital Affair Premium Member Content

The Full Arch Patient, A Fully Digital Affair
Guided Surgery (GS) is a concept where you are guided by the rehabilitation needs of your patient to perform the implant surgery procedure based on all the info that we process via planning software that are loaded with digital files coming from intra-oral scanning (IOS) and cone beam computed tomography (CBCT). After planning the case, we print a surgical guide or stent to perform the surgery, this technique allows us to do it flap or flapless in a complete secure environment, quicker and more precise if we compare to "freehand" surgery. However, we have to be careful because not all the systems are the same; those who use scoops as reductors are less precise than the "scoopless", no matter which system we use we have to be clear to understand that all the classic concepts of osseointegration are not erased by guided surgery, especially bone physiology.

Management of The Critical Size Defect; New Age Tools and Techniques Premium Member Content

Management of The Critical Size Defect; New Age Tools and Techniques
This presentation highlights the placement of 2 implants in a large 3D Vertical & Horizontal defect in the anterior maxilla. The defect is treated by utilizing a mix of autogenous bone harvested using Auto-bone collector and cortical-cancellous particulate Allograft. The mix is then secured by a customized 3D pre-formed titanium membrane followed by "labial contour" augmentation of the defect which is then secured by a rigid absorbable collagen membrane to create and maintain space. This will protect the graft and help stabilize the blood clot during the critical healing time. This membrane is fixed by bone tacks for stability. The final results showing good healing of the soft and hard tissues is displayed.

Negative Factors for Soft & Hard Tissue Maintenance Premium Member Content

Negative Factors for Soft & Hard Tissue Maintenance
Maintaining the bone is the most difficult challenge in implantology (bone grafted or native bone around implants). If a tissue want to live long, it has to follow 2 conditions: The first condition is to organize a full blood supply.. However, it’s not enough. The solution for the long term stability is to try to organize the stability of the blood supply.. by the respect of several biologic conditions. Almost of these conditions are explained in this lecture. We introduce here the new concept to avoid the reduction of blood supply by the periosteal incision: the soft brushing technique is the first technique which allows a very large increase of the flap without any incision: the flap closure without tension but without any incision.

Esthetic Tissue Reconstruction Around Implants - Part 2 of 2 Premium Member Content

Esthetic Tissue Reconstruction Around Implants - Part 2 of 2
Esthetics in implant-supported restorations is an important clinical objective in contemporary dentistry. Understanding the biological behavior of hard and soft tissues following tooth extraction is the first step to anticipate the physiological tissue remodeling and its consequences. Our objective in these 2 sequencial lectures was to provide straightforward decision trees related to several clinical scenarios observed in routine clinical practice. The 3 fundamental pillars for peri-implant excellence are: restorative-driven implant position, hard and soft tissue reconstruction using different biomaterials and grafts, and prosthetic management. Using this philosophical approach seems to provide esthetic and stable results over time irrespective of the initial clinical condition.

Implant Treatment Sequencing

Implant Treatment Sequencing
Periodontal or endodontic related osseous infections of natural teeth may complicate implant treatment. Staging of the surgical aspects of the treatment is important to achieve the desired restorative objectives. Additionally, when cosmetic correction of adjacent natural teeth is required as part of the overall treatment, the final result will be influenced by proper sequencing. Implant surgical placement in immediate extraction sites has been well documented in the literature as a successful treatment…

Flapless Postextraction Socket Implant Placement in the Esthetic Zone: Part 1. The Effect of Bone Grafting and/or Provisional Restoration on Facial-Palatal Ridge Dimensional Change

Flapless Postextraction Socket Implant Placement in the Esthetic Zone: Part 1. The Effect of Bone Grafting and/or Provisional Restoration on Facial-Palatal Ridge Dimensional Change
The dental literature has reportted vertical soft tissue changes that can occur with immediate implant placement, bone grafting, and provisional restoration ranging from a gain or loss of 1.0mm. However, little is known of the effects of facial-palatal collapse of the ridge due to these clinical procedures. Based upon treatment modalities rendered, an ensuing contour change can occur with significant negative esthetic consequences. The results of a retrospective clinical cohort study evaluating the change in horizontal ridge dimension associated with implant placement in anterior postextraction sockets are presented for four treatment groups. Therefore, it is recommended to place a bone graft and contoured healing abutment or provisional restoration at the time of flapless postextraction socket implant placement.

CT-Derived Model-Based Surgery for Immediate Loading of Maxillary Anterior Implants

CT-Derived Model-Based Surgery for Immediate Loading of Maxillary Anterior Implants
A novel approach to predict implant placement utilizing CT-derived model-based surgery was performed for a maxillary-anterior aesthetic immediate extraction/immediate restoration; root proximity and inter-implant positioning was an additional complication. The use of state-of-the-art diagnostic and treatmentplanning tools described in this article has been shown to be effective for both partial and completely edentulous patient presentations. These tools can serve as a foundation for accurate, highly practical, and repeatable CT-scan–derived modelbased presurgical prosthetic planning for the immediate loading of dental implants.
