“Bernotti V-Y Flap”: A Minimally Invasive Recession Coverage Technique

Description:
The Bernotti V-Y Flap, it’s a coronally advanced flap technique with a “V-Y’’ flap design, which consist in a microsurgery technique with or without connective tissue graft, that will improve mucosal biotype and keratinized band in a predictable way for isolated mucosal recession, dehiscence and translucent tissue, at teeth or implants sites. It is a conservative flap design, inspired in Semilunar and Modified Semilunar Tarnow’s technique, to minimize interproximal crestal bone loss and possible loss of papillae around teeth or implants, marginal mucosal position is stabilized avoiding adverse aesthetic outcomes. At least (two) 2mm of keratinized mucosa must be left on each side of the flap to preserve blood supply and avoid loss of crestal marginal bone. Miller Class I and II recession defects with a minimum length of (two) 2 mm are suitable to be selected to perform the Bernotti V-Y Flap; The Bernotti V-Y Flap consist of a pedicle flap, making a ¨V ¨incision apically to the recession defect. The incision ends on each inter-proximal area of the tooth or the implant, but not all the way to the tip of the papilla. Afterwards a mid-facial intra-sulcular incision is made, to allow a coronal movement of the marginal tissue with no tension. The flap is coronally maintained, when it is sutured the vertical slope of the ¨Y¨ incision. Pushing or advancing coronally the flap to cover the mucosal recession without making any tension to the pedicle flap, avoiding any incision or suture at the papillae that could compromised blood supply. The Bernotti V-Y Flap is recommended in the presence of a Keratinized mucosa band or in combination with tissue graft, either on teeth or implants sites. In order to cover isolated mucosa recession, treat dehiscence, translucent tissue and improve mucosa biotype. Stability and aesthetic outcomes are achieved with the Bernotti V-Y Flap.

Excessive Gingival Display Managed by Surgical Lip Reposition, Aesthetic Crown Lengthening and Veneers
Excessive gingival display can be managed by a variety of treatment modalities, depending on the etiology. Mild and moderate gummy smiles can be successfully treated by periodontal plastic surgery, however for excessive gingival display as in cases of VME II or III with 5 to 8 mm or more of gingival exposure this procedure is not enough. Many patients decline the more invasive Le Fort osteotomy and opt for a less invasive surgical lip repositioning procedure. Currently Botox is used as alternative, but this is only a temporary treatment with very short effectiveness. That is why we suggest surgical lip repositioning as an effective procedure to reduce gingival display by positioning the upper lip in a more coronal location. The long-term stability of the results remains to be seen, but it holds promise as an alternative treatment to more invasive procedures.

A Review of Clinical Applications for Soft Tissue Allografts & Autografts: Solutions for Teeth, Implants, and Dentures
The presentation focuses on the clinical applications for acellular dermal graft (ADG) materials. ADGs have been commonly presented and discussed to provide solutions for treating gingival recession. Compared to autogenous grafts, they have the advantage of minimizing pain, discomfort, and surgery time for patients by avoiding an intraoral donor site such as the palate. In addition to treating gingival recession, ADGs also have uses in bone grafting surgeries such as socket grafting and ridge augmentations (GBR) when they are utilized as a barrier membrane. Finally, ADGs can be combined with bone grafting materials to aid in the augmentation of edentulous ridges to aid denture support. While this last application is less common, it's use is worthy of discussion.

During the past two decades, significant advancements with the integration of periodontal plastic surgery into esthetic restorative dental procedures have received increased attention. While ovate pontics have traditionally been used as a restorative design following augmentation procedures to enhance esthetics, an alternate E-pontic design aims to predictably support and maintain the gingival architecture between a single missing anterior tooth adjacent to a natural tooth or an implant that is in harmony with the lip line and face. In addition, the E-pontic design promotes the gingival facial tissue to coronally migrate over the pontic, creating a gingival sulcus.
