The Classic Sinus Lift Window Technique: Shape, Size, Location and Trends

Description:
Since the 1980's articles of Boyne & James descriptive technique, we have been treating the atrophic posterior maxilla almost in the same way. The piezoelectric surgery changed the manner and safety of accessing the schniderian membrane, but in order to establish a predictable result we have to be precise in the three important parameters: the shape, size and location of the lateral access. This is the basis of the procedure, and we introduce here the "Wet Gauge Technique", that makes access safer and cleaner. This method for Schneiderian membrane lift, as well as a new Incision design based on maximizing maxillary vascularity will be introduced . Also, the use of the Clinicae Template for precise implant placement is highlighted as well as the myth of the use of the lateral window wall to be utilized as a new sinus´ floor will be discussed.

Yes, the new incision design places exactly over the sinus window! But don´t worry, the old designs and ideas where developed many years ago, when PRF and microsutures where not available. When we suture mucosa, we get a tight and hermetic closure, that is completely safe. Sinus lift surgery has changed because we understand better Schneiderian bone formation potentiality and biologic cementum like PRF and PRGF help us to seal better incision wounds. Suture materials are better than old silk, and lens magnification also help us to have a better primary healing of our sinus lift procedure.

thanks for the presentation. with the new incision design doesn't this place the incision line very close to the sinus window? in that case u risk loosing your graft in case the incision line opens up for any reason. just a thought. thank u

Sinus Floor Elevation Via the Maxillary Premolar Extraction Socket With Immediate Implant Placement: A Case Series
When immediate implant placement is considered for teeth with close proximity to the sinus floor, apical extension of the osteotomy is significantly limited, and often a staged approach is used. Implant placement into fresh extraction sockets and sinus floor manipulation using bone-added osteotome sinus floor elevation with implant placement are techniques most often used independently or sequentially. In this care report, immediate implant placement with simultaneous osteotome sinus floor elevation is an advantageous combination of two successfully used techniques. This combined approach can significantly reduce the treatment time for implant therapy in teeth with close sinus proximity and provide the operator with the ability to place implants of desired length.
