連続した前歯インプラント:審美的失敗の外科的・補綴的治療 Part4

Description:
Esthetically successful placement of consecutive implants in the anterior region, especially in the lateral/central environment, remains as one of the most challenging endeavors in implant therapy. This webinar will focus on the diagnosis and treatment design options for the implant replacement of two or more adjacent teeth in the anterior region with emphasis on preventing mid-facial and interdental tissue loss. In addition, the surgical and restorative management and correction of esthetic failures will be discussed including the use of connective tissue allograft.

Questions & Comments
Maurice Salama - (7/22/2013 7:48 PM)

Simon and Tom; Tough question. We had bone loss interproximally, we were a little too close to the lateral incisor, and we should have considered a "papilla sparring" incision here. If you review the preop you would notice that this papilla was already reduced by the bone loss in this area. A diagnostic and technique-incision design failure. thanks Dr. Salama

Tom, thanks for bringing us back to a very important subject matter. To answer Simon's question, in my opinion an important factor contributing to the failure of that 1st case is related to not factoring in the deficiency present at the inter proximal height of bone on the mesial of the lateral; look at the pre-implant slide, as well as intra-surgical. That deficiency should have been factored into a) factoring prognosis, b) incision design, and c) Whether to do a 1 stage approach or 2 stage. The corrective procedure performed was necessarily 2 stage and required managing a multitude of surgical and restorative considerations, as well as overcoming the odds.

Thought that the question below by Simon Milbauer was worthy of an answer.

State of the art cases and superb lecture docs!My question is what in your opinion was the reason behind the esthetic failure of the implant in your 1st case?It seemed to be a little bit close to the adjacent lateral incisor on x-ray but when flap was raised the distance appeared acceptable? Many thanks, Simon Milbauer

Maurice Salama - (1/25/2011 9:39 AM)

Samir; If you are thinking about effect on phonetics, as long as their is an air tight seal with the gingiva this has not been a problem at all. good luck Dr. Salama

good result esthetically but what about phonation

Bone Graft Solutions in the Treatment of a Defective & Deficient Ridges; Advantages of Combined Therapy
More patients are treated with implants than ever before. Unfortunately, many have insufficient bone available to support an implant prosthesis. This presentation describes the utilization of several different augmentation approaches for the severely resorbed ridge and dentally handicapped fully endentulous patient. Current concepts related to autogenous bone harvest, 3D bone augmentation combined with blood born bioactive modifiers and digital planning are defined. Video and high resolution images are utilized to further describe these approaches.

Managing Esthetic Implant Complications Premium Member Content

Managing Esthetic Implant Complications
Dental implant success today is judged not only by osseointegration but also by bone, tissue stability and of course long term esthetic results. Cosmetic predictability can often be difficult to attain, and esthetic implant failures can be multifactorial and patient management issues. Once esthetic implant failures occur, many cannot be fully corrected. Some complications must be addressed by an interdisciplinary dental team. In this summary of case reports, surgical considerations are provided, including cases of facial asymmetry/recession due to facial implant placement or bone loss resulting from technique/treatment failures, as well as papillary deficiencies. Restorative considerations for correcting failures are also discussed.

Surgical Veneer Grafting Protocol: Step-by-Step Utilization in the Esthetic Zone
In the surgical part, it will be described the criteria necessary for successfully utilizing minimally invasive protocols within the esthetic zone and the possibility of placing or not placing a bone graft in the “gap” and the opportunity to use a connective tissue graft to overbuild the site bucco-lingually, performing the Surgical Veneer Grafting Protocol, idea'ed by the Agnini brothers. In the digital prosthetic part, the authors will analyze different cases, highlighting the operational differences that they have developed over time, with the increasing knowledge of new materials and daily advances in new software's.

Anterior Extraction & Implant Placement in a Severely Deficient Site
Extraction and implant placement is a functionally predictable therapeutic modality. Esthetic predictability, however, can sometimes prove elusive. This is especially true for the implant replacement of severely compromised anterior teeth with hard and soft tissue deficiencies within the esthetic zone.

Prosthetic Management of Implants with Different Osseous Levels
The osseointegrated implant has become a standard treatment with a high degree of predictability for patients missing one or more teeth. It is especially recommended when the patient has a clear medical history and normal bone levels in both width and height in the edentulous areas. In constant search for improvement, clinicians find themselves looking into every element relating to the replacement of a missing tooth or teeth: existing bone structure, surgical technique, implant placement, implant…
