Early Implant Placement with Contour Augmentation - Biologic Rationale, Surgical Technique & Long-Term Results

Description:
Today, the timing of implant placement post-extraction in the esthetic zone is considered an important factor which influences the esthetic treatment outcome. One of these options is early implant placement after 4 to 8 weeks of soft-tissue healing. Implant placement is then combined with simultaneous contour augmentation by guided bone regeneration (GBR) to compensate for ridge alterations post-extraction. The purpose of this presentation is to present data and to analyze the stability of esthetic treatment outcomes in patients following single-tooth replacement in the anterior maxilla using the concept of early implant placement with simultaneous contour augmentation. Special emphasis will be placed on assessing the stability of the facial mucosa, since it directly depends on the stability of successful contour augmentation by GBR and to compare it to currently utilized immediate techniques like Dual Zone Therapy (Chu & Tarnow) and Partial Extraction Therapies (Salama & Gluckman).

Regenerative Procedures for Implant Site Optimization: Establishing the Perio-Implant Interface
Emerging concepts in implant dentistry have allowed bone preservation to become a reality, thus reducing the need for extensive augmentation procedures. Nevertheless, clinicians are still confronted with atrophic defects that require complex regenerative strategies for dental implant rehabilitation. This presentation will focus on some of the essential elements for a predictable regenerative protocol in implant site development. A discussion will include the use of defect topography as a guide in choosing a suitable augmentation technique. Several considerations that allow for tension free wound closure will be reviewed. Furthermore, an evidence based approach in graft material selection will be presented. Clinical cases will then be reviewed, to demonstrate how to integrate the presented principles into daily clinical practice.

Current Concepts for the Preservation of the Post-Extraction Alveolar Ridge
Different techniques have been presented in literature in order to preserve the post-extraction site; including the use of barrier membranes, tri-dimensional matrix and bone fillers. An “open-healing” approach is nowadays commonly used, combining a proper management of soft tissues with biological integration of biomaterials.
