Fresh Frozen Bone Bank & Bone Marrow Aspiration: An Alternative to Autogenous Bone

Description:
Bone loss in edentulous jaws negatively influences the functions of the stomatognathic system. Reconstructive prosthetic surgery was advocated to provide suitable prerequisites for implant placement and preserve the noble structures, which at the same time satisfies aesthetic demands. For this reason, bone regeneration procedures become mandatory to create the conditions for prosthetic-driven implant placement.  Among the different techniques, proposed over the years, bone reconstruction with autogenous onlay grafts is the best documented and most versatile procedure, as it can be used to treat the vast majority of defects, irrespective of variables such as type of atrophy and extension of the defect. Clinical results are favorable and stable over time either for the reconstructed bone and for implants placed in those areas . In case of extreme atrophy of the alveolar ridge, extra-oral donor sites generally represent the only option to obtain an adequate quantity of bone for the reconstruction.  However, bone harvesting from the anterior ilium presents some morbidity, other extra-oral donor sites, such as the calvarium is less frequently accepted by patients, and, although extremely rare, complications can be relevant. During the presentation, we will share our 5 years experience using Fresh Frozen Bone Allograft in combination with Bone marrow aspiration ( from the same patient). Our full protocol will be exposed and clinical cases, with histological evidence, will be presented.

George, well stated. This is one of the crucial steps toward a lower morbidity future to patients with these problems....incredible that things like this show up first on sites like Dentalxp. My most sincere compliments to Dr. Juan Alberto and his team. I do know that in a similar manner Dr. Craig Misch has also shown outstanding early results with a similar approach utilizing a "Lazarus" graft using BMP-2 and spinal cadaver bone. regards Maurice

Dr. Ruiz-very interesting biologic treatment of a severely resorbed total edentulous patient. These are the most challenging of all implant patients. Non-grafted alternatives include quad-zygomas and grafted alternatives with onlay grafts as you have shown here are the range of treatments. You have good rationale for your treatment protocols that are based on good science. The most important thing also to discuss is not allowing patients to become so resorbed which is why early diagnosis and treatment of the edentulous jaw is the responsibility of the profession worldwide.

Platelet derived growth factors are now routinely utilized in reconstructive therapy. This presentation describes very detailed and evidence based guidelines for clinicians interested in enhancing their abilities in tissue engineering, especially as it relates to bone augmentation. Interpositional Bone Grafts to Treat the Posterior Atrophic Mandible
This presentation analyzes the interpositional bone graft or inlay technique, discussing the reconstructive surgical approach. Horizontal osteotomy with the interposition of bone in the form of a “sandwich” involves raising a coronal osteotomized segment of the mandible, which is still attached to the lingual periosteum, and interpositioning a block bone graft. This technique guarantees a dual vascular supply to the inlay graft from the lingual periosteum and from the residual bone; it also allows optimum use of the native basal bone, which should be less prone to resorption. The advantages and disadvantages of the inlay technique are compared with other commonly used augmentation techniques in the management of posterior mandibular atrophies.

Autogenous bone grafts have been used for many years for alveolar ridge augmentation and are still considered to be the gold standard for jaw reconstruction. The use of these grafts with osseointegrated implants was originally discussed by Brånemark et al., who used the iliac crest as the donor site. For repair of most localized alveolar defects, however, bone grafts from the mandible offer advantages over iliac crest grafts. These include the proximity of donor and recipient sites, convenient surgical access, decreased donor site morbidity and decreased cost. Reconstruction of a Single-Tooth Traumatic Defect in the Anterior Maxilla Using the Khoury Bone Plate Graft
Trauma to teeth and the dentoalveolar process may result in a ridge defect that precludes straightforward implant therapy of the patient. Typically bone and soft tissue augmentation of the area would first be needed to adequately prepare the tissues for the implant and its restoration..Grafting of the site is substantially more difficult in cases where the ridge also lacks adequate height, and techniques to recreate a bony envelope to apply guided bone regeneration may be required. Moreover, defects in the anterior aesthetic zone that require both bone and soft tissue grafting and a restoration that harmonizes the adjacent pink and white aesthetics may be an even more significant challenge to the restorative team. Hereafter a case of trauma to an anterior maxillary tooth that saw destruction of the ridge is presented, with the defect reconstructed to accommodate a functional and aesthetically pleasing implant supported restoration.
