Simon One incision in periostium and blunt stretching the fibers was enough

Great case. do you have post op xrays? how much you had to release to get primary closure

Hi Masoud, due to low density of bone and a very thin buccal bone i didnt do the decortication in this case.

Masuod Adeli - (9/2/2015 5:54 AM)

Great execution Omid, just a question: is there any need for decortication in such cases? thanks Masuod.

Hi Saverio In these areas the quality of bone is D3 so its expandable ,with this design of dental implant its possible to expand the bone simultaneously and instead of removing bone ,we can save more bone between the threads.also as u see the narrowest part is the first 2-3mm of crestal areas and the platform of the implant as i showed in the video does not have any threads in the first 2mm so will save more bone at the crestal part.we definitely have a thinned buccal plate especially at the crest but have minimized the risk of dehiscence .depth between threads increases from platform toward apical third and again decrease toward the apex of implant.

SAVERIO ACCARDI - (8/24/2015 12:19 PM)

Hi Omid, can you clarify it: on the CT scan the bone width where you are going to place the implants is approximately 3.4 mm , how can you place implants of diameter 4 and 5.5 with no bone dehiscence ??

Omid Moghaddas - (8/22/2015 2:14 PM)

Joey ,in some cases i didnt see any fibrous layer but in a few cases that i did there was a very thin layer.

Omid Moghaddas - (8/22/2015 2:13 PM)

Thanks Maurice.yes it can be used for many systems like ITI,Nobel biocare,Dio,Dentium,Zimmer ,ankylos,... According to the product broucher there are 4 types of flat abutment that each is compatible with many systems.

Maurice Salama - (8/22/2015 1:40 PM)

Great job Omid. Is this membrane implant specific or can it be used with other systems? regards Dr. s

Surgical Protocols for Socket Type 2 in the Aesthetic Zone
The topic of this presentation is an implant surgical procedures for a socket type 2 (socket missing buccal bone) in an aesthetic area, with emphasis on modified IVAN* technique for socket preservation. IVAN* stands for inter-positional vascularised augmentation neogenesis. Author`s guidelines for selection of appropriate surgical protocols for sockets missing buccal bone wall, protocols that are proved to be most effective, are presented. Early implant placement with GBR, prosthetic socket preservation and socket augmentation with socket sealing are shortly presented as alternatives for some cases. From author modified IVAN technique is shown step by step.

Emphasis will be placed on the benefits of the augmentation technique on revascularization: the use of autologous bone grafts on a scaffold of xenograft material saturated in PRP (Platelets Rich Plasma), covered with PPP (Platelets Poor Plasma). Soft tissue manipulation with role-up technique is demonstrated prior to implants placement.
Six months after implants placement, the second phase of uncovering is shown.

Bone Reconstruction: A New Algorithm for the Implantologist
Bone augmentation is often required to place an adequate number of dental implants in ideal positions for prosthetic support. In addition to biomechanical demands bone augmentation can also provide proper ridge contour for improved esthetic outcomes. Disadvantages of bone augmentation techniques include increase morbidity, surgical time, costs and treatment length. As such a trend has develop towards treatment approaches that avoid bone grafting including shorter and narrower implants, angled implants and/or fewer implants for prosthetic support. This presentation will look at various bone augmentation techniques, methods to minimize the morbidity of bone grafting and guide clinicians on implant rehabilitation with grafting versus non-grafting approaches.
