Immediate Molar Implants: When, Why, and How?

Description:
Multi-rooted extraction sockets are often too large to accommodate a traditional diameter implant, therefore ultra wide diameter implants as a clinical alternative can predictably simplify procedures without compromising function and esthetics. Problems with use of traditional implants may include the ability to gain adequate primary stability, use of large amounts of graft material, and poor embrasure spaces causing food impaction. The focus of this lecture will highlight clinical rational to improve success, anatomical and restorative benefits of immediate placement, evidence-based outcomes reported in literature, and survival and success rates reported by the presenter. This lecture is intended for restorative dentists.

This was such a well organized and defined presentation on a specific topic. My only question is related to long term review of wide diameter implants and their effect of labial bone remodelling after load and time....thnx Dr. Salama

Emerging concepts in implant dentistry have allowed bone preservation to become a reality, thus reducing the need for extensive augmentation procedures. Implant placement in extraction sockets is considered a daunting challenge due to offering risks of bone remodeling and consequent gingival alterations. Objective: This study aims at proposing a protocol for selecting the diameter of upper anterior osseointegrated implants placed in extraction sockets. This protocol was based on the bucco-palatal dimension of the socket and allows a 3-mm gap to form between the implant and the outer surface of the buccal bone wall. Such gap must be filled with biomaterial or autograft so as to increase the predictability of long-term results.

Does residual cement around implant-supported restorations cause peri-implant disease? A retrospective case analysis
The purpose of this study was to determine the relationship between patients with a history of periodontitis and development of cement-related peri-implant disease. Seventy-seven patients with 129 implants for this retrospective analysis were selected from completed implant cases that were scheduled for regular maintenance or had experienced mechanical or biological complications between years 2006 and 2011 in private practice. Implants with extracoronal residual cement and implants without cement remnants were analyzed.
