Implant Placement in the Posterior Mandible in Combination with Titanium Mesh and PRGF for Horizontal Bone Augmentation

Description:
In this video, Dr. Salama highlights a combination case where implant placement is performed simultaneously with a pre-fit titanium mesh, FT Wing to secure spacing needed for ridge augmentation. PRGF and fibrin membrane further highlight this regenerative protocol. Incision and tension free closure are also featured.

Questions & Comments
Maurice Salama - (8/27/2014 4:28 AM)

Ed; My goal was 2mm labial to the implant. I now use a mixture of 50:50% Cortical-Cancellous Mineralized Bone or I collect autogenous bone with the ACM (Auto Chip Maker) drills. Dr. Salama

edward shapiro - (8/26/2014 10:54 PM)

dr. salama. what was your goal in mm in regeneration of the buccal bone. i use only mineross for my GBR procedures; should i be using straight cortical bone on the outer aspect; a layering technique? sounds like more bone regeneration that way. thanks.

Maurice Salama - (12/10/2013 10:57 AM)

Kevin; The best way is to purchase a Neo-GBR kit from Neobiotech and then purchase the CTi-mesh separately. Secure screws on both buccal and lingual and a tenting screw below the mesh to maintain the space. You can purchase these items directly on the Products page and click on Neobiotech. Good luck Dr. S

I want to use the cti mesh from neobiotech to get some vertical height regenerated. but i want to stage it. what size screws would you recomend to secure it to the buccal and lingual? or would i just need to secure it to one side?

Maurice Salama - (10/17/2013 9:26 AM)

Mark; I have not utilized Cytoplast Ti reinforced for this procedure. I would be reluctant to as it does not allow for the same space maintaining as does Titanium mesh only. I too think Cytoplast membranes are good options for buccal dehiscence of an extraction site but I also like the Socket repair membrane of Zimmer for this indication. regards Dr. Salama

Mark Zwickey - (10/16/2013 11:00 AM)

Dr. Salama - Have you ever or do you know of any techniques similar to this but using titanium reinforced cytoplast? I have thought about this in the past. Using the cover screw or low profile healing abutment to secure the cytoplast. That way if there is some breakdown over the cover screw, which is more of a problem in the lower, the exposure may be tolerated better. I have had good success using that membrane for buccal dehiscence after ant extraction and not getting primary closure, but have not had great success around implants. Would securing with the screw help with stability and better results? I am thinking about using primarily when you have a small buccal exposure of the implant on the lower but good primary stability otherwise. thanks.

Dr. Eldardir; Thank you. It is best to extend the envelope flap full thickness in the area until you can see the exit of the nerve from the foramen reaching out into the flap. An depression in the labial wall typically extends beyond the nerve. Due this with blunt dissection only. This is dispalyed beautifully on Dr. Pikos video on XP for mandibular block graft. Take a look. Good luck Dr. Salama

Islam Eldardir - (7/18/2012 10:16 AM)

hi doc maurice could you show us how u visualize the mental nerve and keep it away i always have the fair of going near this area and try to be superfacial as much as i can plz. thank alot

Simon Milbauer - (10/22/2011 3:00 PM)

Maurice, thank you for your answer and this another fantastic case presentation. No,I haven't tried the PRGF techniques as yet.I see it is a routine practice for most of the experts on Dental XP and the results are excellent.

BMP, PRP, PRGF and PRF; At the Edge of Regenerative Surgery
Biological modifiers and growth factors are substances that the body produces naturally in response to injury or disease. The use of these substances in regenerative procedures has made hard and soft tissue regeneration more predictable while accelerating healing and decreasing patient morbidity. This webinar will review the wound healing process and the role that growth factors play in regeneration. BMP, PRP, PRF and PRGF will be reviewed and their clinical applications will be demonstrated. At the end of this webinar the participant will have a good understanding of the differences between these biological modifiers and their benefits.

Combination Therapies in the Esthetic Zone: Steps for Success
This presentation will cover the critical aspects to consider for deciding the sequence and implementation of combination therapies. Successful esthetic zone treatment must begin with a well thought-out space management plan. This plan involves a three dimensional adjustment of the periodontal attachment, with a possibility of root submergence and partial extraction technique. Following this, a decision is made concerning the timing and position of implant placement, and an adequate hard tissue foundation can be restored using GBR with titanium mesh and collagen membrane. On top of this base a more efficient soft tissue augmentation and adjustment completes the requirements for success in the esthetic zone.

Influence of Thin Mucosal Tissues on Crestal Bone Stability Around Implants With Platform Switching: A 1-year Pilot Study
The aim of this pilot study was to determine what effect thin mucosal tissues can have on crestal bone stability around implants with platform switching. Within the limitations of this pilot study it can be concluded that implants with platform switching did not preserve crestal bone better in comparison with implants with traditional implant-abutment connection if, at the time of implant placement, thin mucosal tissues were present.
