In this instructional lecture recorded at a recent symposium in Florida in June 2009, Dr. Nicolas Elian, Assistant Professor and Head of the Division of Implant Dentistry at New York University College of Dentistry, outlines protocols for site assessment and enhancement for successful esthetic implant therapy. The single tooth site, as well as larger edentulous sites are discussed. In addition, a very detailed description is given on the utilization of bone grafting with or without membranes, including the use of titanium mesh, to achieve predictable 3 dimensional enhancement.

Current Concepts for the Preservation of the Post-Extraction Alveolar Ridge
After tooth extraction, bone resorption of varying amounts always occurs as the edentulous site of the alveolar process undergoes both qualitative and quantitative changes. Different techniques have been presented in literature in order to preserve the post-extraction site, including the use of barrier membranes, tri-dimensional matrix and bone fillers. An “open-healing” approach is nowadays commonly used, combining a proper management of soft tissues with biological integration of biomaterials. Prosthetic Soft Tissue Development From Single to Full Arch Reconstruction
In addition to surgical intervention is the creation of the specific emergence profile that is essential in the aesthetic zone. The emergence profile composed of 2 parts, the abutment and the subgingival part of the crown. The shape of abutment can be individually shaped so that it gives natural appearance and varies individually (depending on the depth, angulation and diameter of the implant). Sometimes it even dictates the implant position. From a surgical perspective, soft tissue height, position and thickness need to be diagnosed and corrected when needed. From the prosthetic point of view, the emergence profile has to be created to mimic the natural appearance and maintained over time in respect to the biological changes. The course teaches step by step how to be successful with implant prosthetics from single tooth, partially edentulous to full arch reconstruction.

Bone Augmentation vs Short Implants: Making Clinical Decisions for Tooth Replacement at Sites with Alveolar Ridge Atrophy
There is often some degree of atrophy in sites planned for implant placement. An array of therapeutic options is available for implant placement in sites with vertical atrophy of the alveolar ridge, including ridge augmentation, sinus augmentation in the maxilla, nerve repositioning in the mandible and tilted implants. Short implants have been systematically evaluated as an alternative in sites with vertical alveolar ridge atrophy with favorable clinical outcomes comparable to standard-length implants. Decision making for selection of appropriate therapeutic modality for sites with vertical alveolar ridge atrophy has to consider many theoretical, surgical and prosthetic considerations. Surgical and prosthetic guidelines will be provided for application of short implants for posterior sites with limited vertical height.

Success Rate, Healing Time, and Implant Stability of Photofunctionalized Dental Implants
This is the first study to report the clinical outcomes of photofunctionalized dental implants. This retrospective study analyzed 95 consecutive patients who received 222 untreated implants and 70 patients who received 168 photofunctionalized implants over a follow-up period of 2.5 years. Photofunctionalization was performed by treating implants with UV light for 15 minutes using a photo device immediately before placement. Within the limits of this retrospective study, despite the more frequent use of shorter and smaller-diameter implants, the use of photofunctionalization allowed for a faster loading protocol without compromising the success rate.
