Interpositional Bone Grafts to Treat the Posterior Atrophic Mandible

Description:
This presentation analyzes the interpositional bone graft or inlay technique, discussing the reconstructive surgical approach. Horizontal osteotomy with the interposition of bone in the form of a “sandwich” involves raising a coronal osteotomized segment of the mandible, which is still attached to the lingual periosteum, and interpositioning a block bone graft. This technique guarantees a dual vascular supply to the inlay graft from the lingual periosteum and from the residual bone; it also allows optimum use of the native basal bone, which should be less prone to resorption. The advantages and disadvantages of the inlay technique are compared with other commonly used augmentation techniques in the management of posterior mandibular atrophies.

Different surgical procedures have been proposed in reconstruction of deficient alveolar ridges for placing dental implants in optimum prosthetic and esthetic position. One of the inconveniences of titanium mesh which has been reported is the exposure of it but on the other hand the benefit in comparison to non resorbable membranes is that there is no need for removal and the risk of infection is very low. In this lecture different cases will be presented and clinical points to be considered in using titanium mesh will be highlighted.

Biphosphonates Related Osteonecrosis of the Jaw is one of the tragedies affecting our patients each time more often representing a new challenge in what we like to call human dentistry. This severe and unpredictable outcome can evolve from something as simple as a tooth extraction or simply an implant placement. In this presentation the authors display one of the first BRONJ cases in the world treated with the surgical biosealing technique using the PRGF ENDORET graft. Understanding the BIOlogy behind the healing process has been as much surprising as useful in trying to alleviate our patients pain and improve their quality of life. This video presents a novel technique to the post-extraction site based upon taking care of the overall tissue support from the bone level and we will discuss treatment planning, options and limitations with this technique/protocol.

The loss of alveolar ridge contour in the esthetic zone compromises both esthetics and function. This clinical presentation will focus on the application of both hard and soft tissue grafting in the esthetic zone implementing state of the art interactive CT diagnosis and treatment planning for optimal esthetic implant reconstruction. Emphasis will be on indications, graft and harvest site assessment, timing, and use of bioactive modifiers including BMP, PRGF and PDGF. The surgical protocol utilizing allogeneic and autogenous grafts in conjunction with connective tissue grafts, acellular dermis matrix, and related soft tissue procedures to avoid functional and esthetic pitfalls will be featured. Advanced Bone Grafting Techniques: Part 1 - Autogenous Bone Blocks in the Reconstruction of the Atrophic Mandibular Ridge
This lecture will describe the use of autologous onlay block bone grafting for reconstruction of moderate to severe atrophic posterior mandibular alveolar ridge. This will include a discussion of the various options to this form of grafting, such as Short implants, Lingual positioning, nerve re-positioning, distraction osteogenesis, and GBR. The Mutli-Tier onlay Bone Graft Technique will be highlighted in this presentation. Additionally, a review of a recently submitted research paper of long term follow up of implants placed into autologous bone blocks harvested from the mandibular ramus, retromolar and sympheseal areas will be discussed. A description of surgical donor harvest sites in the mandible will also be performed as well as post op healing of these sites. Additional time will be spent reviewing causes of failure with bone blocks and implants placed into these sites.

Physical And Chemical Properties Of Commercially Available Mineralized Bone Allograft
Bone graft materials are critical to the success of dental implants when there is a need to increase the volume of bone in a defect. The surface properties of these graft materials will have a profound impact on the outcome of the graft procedure. The clinician has many choices of bone graft substitutes when augmenting bony deficits. Allograft bone is the most widely used class of bone graft substitutes. Within this class there are a number of different bone allografts, which are manufactured utilizing widely varying processing techniques. There also appears to be a wide range of results in the published literature across the spectrum of different bone allografts. This in-vitro study evaluated chemical and surface properties of five different commercially available mineralized bone allografts.
