There is rarely a day that clinicians do not have to deal with an extraction site. If implant therapy is a consideration, then bone and soft tissue quantity and quality must be factored into treatment options. The ideal opportunity for bone preservation exists at the time of tooth extraction. All too often this opportunity is lost. Extraction techniques will be demonstrated that not only preserve but augment the amount of alveolar bone. The importance of piezosurgery, atraumatic surgical technique, the use of bone grafts, resorbable membranes and growth factors will be elucidated along with soft tissue enhancement.

can you tell me the indications of guided gingival growth and when you can use that over subep ct grafting? and the indications for both. thanks

Guided gingival growth is used to augment the amount of tissue at the time of second stage surgery. it allows you develop additional tissue without harvesting a CT graft. Of course you could always use a CT graft in addition or instead of Guided Gingival Growth(ggg). I used ggg when I need 1-2 mm of additional tissue and I do not want to submsect the patient to an additional procedure.

Dear Michael, could you explain the rational behind which you mix your Demineralized dried bone with still water and not sterile saline?

I use sterile water not saline, because I mix the Gem 21 with the hydrated graft material and the saline may affect the efficacy of the Gem 21.

vijailakshmi acharya; It is a non-autologous recombinant rhPRGF sold by Osteohealth in the USA. It is by definition a bioactive protein (highly purified recombinant human platelet-derived growth factor, rhPDGF-BB) then mixed with an osteoconductive matrix carrier (beta tricalcium phosphate, β-TCP). I am NOT a big fan of this product. Sam

Bone augmentation is often required to place an adequate number of dental implants in ideal positions for prosthetic support. The 3 fundamental pillars for peri-implant excellence are: restorative-driven implant position, hard and soft tissue reconstruction using different biomaterials and grafts, and prosthetic management. Using this philosophical approach seems to provide esthetic and stable results over time irrespective of the initial clinical condition.

Long Term Follow-Up of Dental Implants Placed in Autologous Onlay Bone Graft
The aim of this study was to evaluate the efficacy of autologous intraoral onlay bone grafting (OBG) in correlation with long-term survival rates of dental implants placed in the augmented bone. A retrospective study was conducted on 214 patients who received a total of 633 dental implants placed in 224 autologous intraoral block OBG augmentations, combined with Bio-Oss – mixed with platelet-rich plasma (PRP) and covered by platelet-poor plasma (PPP) – as scaffold, with a follow-up time up to 137 months. We suggest that augmentation of severely atrophied jaw bone through the placement of horizontal and/or vertical intraoral OBGs in combination with Bio-Oss saturated with PRP and covered by PPP should be considered a reliable, safe, and very effective surgical technique for obtaining high bone graft survival rate and high long-term implant survival rate.
