Maxillary Molar Extraction and Immediate Implant Placement with Crestal Sinus Lift

Description:
Atraumatic maxillary molar extraction via root sectioning followed by hydraulic crestal sinus lift and immediate implant placement. Sinus lift performed at interradicular area with sinus membrane elevated approximately 3 mm with specialized kit using saline injected through osteotomy. Hydraulic pressure gently raises the sinus membrane without risk of perforation.

Good work! I'd like to know why didn't you use FGG into the removed tooth area or another soft-tissue manegement? I think it is risky to hold the membrane open still!?It's one thing if the sutures are not stable and therefore the membrane is stripped.The other lead is initially wound with an open membrane(though non-resorbable PTFE) Thanks.

Dr. Imran... the membrane is non-resorbable PTFE, and does not need to be covered. As long as it is not contaminated when placed, it usually does not become infected. It is removed in one month.
Dr. Potocsky...I showed this case because the large intraradicular area allowed simultaneous sinus lift, and it became an all-in-one video. That large of an area of bone is not seen routinely.
Typically in a maxillary molar, the tooth would be extracted and just socket grafting performed. Then in four to five months, a crestal lift is done and implant placed.
I don't like placing implants into the palatal socket, since the angulation of the abutment relative to the implant becomes severe.
Dr. Zidile...this was a referred case to me so I don't have follow-up restorative photos yet. I will try to get them and post them.

Minimally Invasive Transcrestal Sinus Augmentation(MITSA)
In this video Dr Kher discusses a method of transitioning from the lateral window sinus elevation procedure to a less invasive transcrestal sinus elevation in cases with alveolar ridge height between 3-5mm. The technique is based on using an ossteotome mediated lift combined with a novel delivery system of injecting the calcium phosphosilicate putty graft material. Dr Kher presents a case series wherein lifts in excess of 10mm were consistently achieved using this technique. A study comparing the increase in alveolar ridge height with the novel transalveolar technique and the conventional lateral window lift is also a part of the presentation.

Alternative Applications of Guided Surgery: Precise Outlining of the Lateral Window in Antral Sinus Bone Grafting
Computed tomography (CT) and the application of CT-based guided implant surgery allow clinicians to provide enhanced precision and accuracy in implant surgery. Because of the difficulty in transferring a patient’s often complex anatomic sinus configurations, as viewed on a preoperative CT scan, into precise osteotomy cuts at antral bone graft surgery, a prototype cutting guide was developed. The surgical guide was developed through the use of CT imaging and the stereolithographic process to precisely position the lateral window, facilitating schneiderian membrane elevation. This report demonstrates the step-by-step method to perform precise guided sinus window preparation using computer software and a stereolithographically generated surgical guide.

Sinus Floor Elevation Via the Maxillary Premolar Extraction Socket With Immediate Implant Placement: A Case Series
When immediate implant placement is considered for teeth with close proximity to the sinus floor, apical extension of the osteotomy is significantly limited, and often a staged approach is used. Implant placement into fresh extraction sockets and sinus floor manipulation using bone-added osteotome sinus floor elevation with implant placement are techniques most often used independently or sequentially. In this care report, immediate implant placement with simultaneous osteotome sinus floor elevation is an advantageous combination of two successfully used techniques. This combined approach can significantly reduce the treatment time for implant therapy in teeth with close sinus proximity and provide the operator with the ability to place implants of desired length.
