Modern Approach of Full Arch Immediate Loading

Description:
In Immediate loading, the survival rate is often based on the follow-up of the osseointegration, but without aesthetic appreciation... The main goal is to achieve osseointegration and aesthetics in order to get long term stability. For this goal we need to obtain both, bone formation and gum thickness, as any failure in one of the 2 factors, will induce the loss of the second factor. Several requirements must to be met in order to obtain and to maintain them together for a very long time: - Biological requirements are linked to the early vascularization (appreciation of the biologic level, growth factors, collagen environment) - Anatomical & surgical requirements will help to the best implant placement and positioning (Fast guide, sub-crestal insertion, platform switching etc..) - Prosthetic requirements are essential for aesthetics as a guidance for soft tissue design (rigid and screwed provisional prosthesis) This presentation will show my statistics about more than 1100 full arch loaded immediately since 14 years (> 7700 implants)

Great presentation.. I like what I see in the design of the Tekka implant .. are they distributed in the United States? Also Wondering if the fast guide is sold in the states.. the gmail contact information that was given as a source was a dead end for me and a internet search was not productive either

Anterior maxillary alveolar bone deficiency, can prevent implant placement for fixed rehabilitation and jeopardize the esthetic outcome. Clinical case of severe atrophy of anterior maxilla, due to loss of implants, is described: A combination of sub-nasal procedure and intra-oral autologous onlay bone graft were used for ridge augmentation and dental implant placement in steps procedure, combined with Bio-Oss saturated in Platelets Rich Plasma (PRP) and covered with platelets-poor-plasma (PPP) as a "biological membrane".

Bone augmentation is often required to place an adequate number of dental implants in ideal positions for prosthetic support. Atrophic Posterior Maxillan and Mandible: Alveolar Ridge Reconstruction with Mandibular Block Autografts
Autogenous bone grafts have been used for many years for alveolar ridge augmentation and are still considered to be the gold standard for jaw reconstruction. The use of these grafts with osseointegrated implants was originally discussed by Brånemark et al., who used the iliac crest as the donor site. For repair of most localized alveolar defects, however, bone grafts from the mandible offer advantages over iliac crest grafts. These include the proximity of donor and recipient sites, convenient surgical access, decreased donor site morbidity and decreased cost. This article focuses on posterior maxillary and mandibular osseous augmentation in a staged approach for implant placement. Both horizontal and vertical deficiencies are addressed with the use of symphysis and ramus buccal shelf donor block bone via case presentations.

Maintenance of Alveolar Ridge Dimensions Utilizing an Extracted Tooth Dentin Particulate Autograft and Platelet-Rich Fibrin: A Retrospective Radiographic Cone-Beam Computed Tomography Study
This study utilized radiographic comparative analysis in order to evaluate dimensional ridge changes four months after tooth extraction and immediate grafting with mineralized dentin particulate autograft and chopped platelet-rich fibrin. Fifty-eight extraction sockets with up to 2mm of missing buccal bone in the coronal aspect compared to the lingual bone were included. Graft material was covered with either a platelet-rich fibrin membrane or collagen sponge with no effort to achieve primary closure.

This report demonstrates the remarkable efficacy of guided bone regeneration using a combination of titanium mesh, resorbable collagen membrane, and rhPDGF for vertical ridge augmentation, thus expanding the indications for implant therapy and allowing recovery of the three-dimensional esthetic architecture in a severely absorbed alveolar ridge.
