Modern Crown Lengthening: Classification and Clinical Application

Description:
Crown lengthening procedures are indicated to provide adequate tooth structure in case of subgingival tooth fracture or caries, uneven gingival level, un-esthetic short crowns due to the tooth wear, inadequate axial height, altered passive eruption ,forced eruption of a single or multiple teeth and finally in case of gingival smile. The treatment modality for esthetic crown lengthening procedure should be based on detail diagnosis in each case because of the multifactorial etiology and because of the type of therapy selected by the clinician, will have a direct implication for the esthetic result.

Clinicians must be aware of patient expectations when planning dental treatment. When full mouth reconstruction is planned, it is very important to satisfy such expectations in order to provide successful treatment. Patients are often concerned about the cost and duration of treatment, and the pain, esthetics, comfort, and function after treatment. However, we cannot assure longevity unless the treatment has a scientific basis. During reconstruction or restoration, we generally remove and replace the existing crowns. Secondary caries are mostly detected on removal of these. Due to the caries, the ferrule and tooth thickness are not adequate; and frequently, the condition may not be suitable for prosthetic treatment. In such cases, we choose to extract the tooth or perform crown lengthening to retain the tooth.Thse days many teeth that we could retain by crown lengthening procedure are replaced by implants. Tissue Engineering and Platelet Derived Growth Factors: Evidence Based Therapy
Platelet derived growth factors are now routinely utilized in reconstructive therapy. This presentation describes very detailed and evidence based guidelines for clinicians interested in enhancing their abilities in tissue engineering, especially as it relates to bone augmentation. Specifically, while successful bone augmentation requires the standard surgical parameters of space maintenance, low pressure on the grafts and tension-free flap closure, optimization of this goal requires management and enhancement of the local biological conditions with growth factors. Towards that end, platelet concentrates through the constant release of growth factors are able to promote and enhance new vascularization, provide plasma protein, normal lipidemia, as well as increased collagen and fibrin activity.

Implant therapy is in the age of being increasingly aesthetically driven, not merely restorative. With the increasing popularity of immediate implants, particularly with anterior tooth extraction, the relevance of socket changes following extraction has come to the fore. Contrary to what was originally believed, the installation of an immediate implant does not arrest bone loss, and immediate implants in the aesthetic zone are prone to recession of bone and soft tissue as a result of bundle bone resorption which is obligatory with tooth extraction.

The ideal opportunity for bone preservation exists at the time of tooth extraction. All too often this opportunity is lost. Extraction techniques will be demonstrated that not only preserve but augment the amount of alveolar bone. The importance of piezosurgery, atraumatic surgical technique, the use of bone grafts, resorbable membranes and growth factors will be elucidated along with soft tissue enhancement.

The Pontic-Shield: Partial Extraction Therapy for Implant Dentistry
Augmentive ridge preservation techniques aim to manage the postextraction ridge. The partial extraction of teeth may better preserve the ridge form by maintaining the bundle bone-periodontal tissues and preserve the ridge beneath dentures or fixed prostheses. The socket-shield technique entails preparing a tooth root section simultaneous to immediate implant placement and has demonstrated histologic and clinical results contributory to esthetic implant treatment. A retrospective 10-patient case series treating 14 partial extraction sites demonstrates how a modification of the socket-shield technique can successfully develop pontic sites and preserve the ridge.
