In this second of a two part presentation, Dr. Edward Mills outlines the rationalization and parameters that guide the utilization of narrow diameter implants in the aesthetic zone. In addition to 3.0 mm 2-piece implants, Dr. Mills also uses surgical videos to demonstrate the use of one-piece implants to overcome aesthetic challenges related to lack of bone and soft tissue dimensions.

Nice job with the custom abutment preparations. Using the jigs for this was a good feature. I have a lady who is much like the mandibular case you showed....a tall thin ridge which is narrower at the apical portion of orthodontic manipulated incisors. She is going to lose at least two of the incisors #'s 23 and 24. She will also lose #28 due to internal resorption and has only 9 mm of bone vertical above the foramen. She does not wish to have onlay grafts to widen the ridge, so we are most likely going to tell her she requires a long span conventional bridge from #21 to 28 with 28 being a pontic. We had considered an attempt to use 1.8 mm imtec implants, but in the apical portion the angulation is not good. These cases certainly can be a huge challenge

Nice job Dr Mills. Are you only suggesting these 3.0mm implants as single units in small spaces or can they be used as standard abutments for splinted cases of fixed or removeable prosthetics? thanks Band

Long Term Follow-Up of Dental Implants Placed in Autologous Onlay Bone Graft
The aim of this study was to evaluate the efficacy of autologous intraoral onlay bone grafting (OBG) in correlation with long-term survival rates of dental implants placed in the augmented bone. A retrospective study was conducted on 214 patients who received a total of 633 dental implants placed in 224 autologous intraoral block OBG augmentations, combined with Bio-Oss – mixed with platelet-rich plasma (PRP) and covered by platelet-poor plasma (PPP) – as scaffold, with a follow-up time up to 137 months. We suggest that augmentation of severely atrophied jaw bone through the placement of horizontal and/or vertical intraoral OBGs in combination with Bio-Oss saturated with PRP and covered by PPP should be considered a reliable, safe, and very effective surgical technique for obtaining high bone graft survival rate and high long-term implant survival rate.
