Osteotome Technique & Piezosurgery Utilized for Internal Sinus Lift with PRGF Protocol

Description:
In this video, Dr. Salama highlights multiple regenerative tools and protocols in the posterior maxilla. Internal sinus lift utilizing a combined osteotome technique and PRGF is featured first prior to implant insertion and followed by bone grafting of the thin labial plate using PRGF, fibrin, and 50:50 mixture of mineralized irradiated allograft bone, Miner-Oss and xenograft. Hydraulic effects of Fibrin and PRGF for sinus lift are also featured along with autologous fibrin membrane utilization to protect buccal bone augmentation.

Abdusalam; That was certainly another option here. I did not feel it was needed as we had adequate length of KT and bone graft for contour seems enough. See the thickness at early healing at the end of the video. regards Dr. Salama

Michael; I also use the NSK unit and it is very good. The tips to use with their system would be the ST-2 and ST-4 but they do not seem to have a osteotomy tip as of yet. good luck Dr. Salama

Michael;
I utilized the "osteotome" tip provided by Piezosurgery and then went to the OT-4 once I make contact with the sinus floor. Hope that helps.

Dear Dr Salama,
I have been using PRGF for last 8 years with very good results. I even visted BTI,Spain in 2005 to know the technique in depth. It is very good technique and cost effective as well. Today I would like use Dental XP platform to convey my difficulties about getting BTI material in India. I even wrote twice to Dr Anitua,but there was no reply. Dr Salama,I would be greatful to you if you direct this message to Dr Anitua. BTI office in Spain didn't respond. I need activator and other material very badly. I hope Dr Anitua will consider my problem and contact as early as possible.

Tissue Engineering and Platelet Derived Growth Factors: Evidence Based Therapy
Platelet derived growth factors are now routinely utilized in reconstructive therapy. This presentation describes very detailed and evidence based guidelines for clinicians interested in enhancing their abilities in tissue engineering, especially as it relates to bone augmentation. Specifically, while successful bone augmentation requires the standard surgical parameters of space maintenance, low pressure on the grafts and tension-free flap closure, optimization of this goal requires management and enhancement of the local biological conditions with growth factors. Towards that end, platelet concentrates through the constant release of growth factors are able to promote and enhance new vascularization, provide plasma protein, normal lipidemia, as well as increased collagen and fibrin activity.

The Effect of Piezoelectric Use on Open Sinus Lift Perforation: A Retrospective Evaluation of 56 Consecutively Treated Cases From Private Practices
The lateral window approach to maxillary sinus augmentation is a well-accepted treatment option in implant dentistry. The most frequent complication reported with traditional techniques has been the perforation of the Schneiderian membrane, with erforation rates ranging from 11% to 56%. The purpose of this retrospective, consecutive case series from two private practices was to report on the rate of Schneiderian membrane perforations and arterial lacerations when a piezoelectric surgical unit was used in conjunction with hand instrumentation to perform lateral window sinus elevations.

Healing in Smokers Versus Nonsmokers: Survival Rates for Sinus Floor Augmentation with Simultaneous Implant Placement
Evidence suggests that smoking is detrimental to the survival of dental implants placed in grafted maxillary sinuses. Studies have shown that improving bone quantity and quality, using rough- surfaced implants, and practicing good oral hygiene may improve outcomes. In this prospective study, the long-term survival rates of implants placed simultaneously with sinus grafting in smokers and non- smokers were compared.
