Maxillary sinus surgery is a reliable and predictable treatment option for the prosthetic rehabilitation of the atrophic maxilla. Nevertheless, these interventions are not risk-less of postoperative complications with respect to implant positioning in pristine bone. The aim of this lecture is to report the results of a clinical consensus of experts (periodontists, implantologists, maxillofacial surgeons, ENT, and microbiology specialists) on several clinical questions and to give clinical recommendations on how to prevent, diagnose, and treat postoperative infections.

Dong thank you for your question, As long as the bone graft is maintained in the closed space, the infection rate is very low. But if you have a perforation into the sinus and the bone is floating inside the sinus that is mainly the cause of the infection. Thank you

Thank you Dr. Tiziano for your great lecture. I like to answer the questions that was presented by Federico and Arthur: 1) Regarding the post op instructions and medications: - Recommend 5-7 days of no strenuous activities, - Augmentin 875 twice a day, - Afrin nasal spray- alpha adrenergic agonist, only for three days - Sudafed ( not prescribed in hypertensive patients) - Usually do not prescribe Flonase. It is a corticosteroid - it will take few days for it to be effective and cannot use it in certain patients

I have two questions related with post-op indications: 1-What if the patient is an active sportman?.How many days of rest should be given before starting with his normal activity(such as running or weight-lifting?in order to avoid complications. 2-Which corticoids do you use as a norm?and how often? thank you again!Federico Villarruel

Thank you for a meaningful and valuable presentation. Is there a use for fluticasone proprionate(Flonase)or other inhalational anti-inflammatories in the case of a blocked ostium, in conjunction with antibiotic administration? Thank you Art Lyford

very good lecture !!

As long as bone graft is used in the sinus, complication can't be avoid. The best way to prevent complications is not to use bone grafts at all in the sinus.
Many studies on graftless sinus augmentation reveals 0% infection. Thank you. DS

Transcrestal Sinus Floor Elevation: Most Frequently Asked Questions
Over 20 years of clinical application and research on transcrestal sinus floor elevation (TSFE) procedures using a variety of implants, graft materials and instrumentation has proven its reliability, and versatility. While improving upon the original osteotome technique they have not yet eliminated the need for a more invasive lateral approach, especially at sites with very limited (1-2 mm) sub-antral bone. Extensive analysis of the existing research and long-term clinical experience will provide the answers to the most pertinent questions surrounding the transcrestal approach including but not limited to: patient selection, clinical limitations, instrumentation, graft selection/necessity and management of membrane perforations.

Sinus Augmentation for Single-Tooth Replacement in the Posterior Maxilla: A 3-Year Follow-up Clinical Report
A technique for single-tooth sinus lift and simultaneous implant placement in the posterior maxilla is presented. Ten hydroxyapatite-coated cylindric implants, 13 to 15 mm in length, were placed together with a composite bone graft of demineralized freeze-dried bone allograft and autogenous bone in 10 adults. Surgical technique and anatomic considerations are discussed. Follow-up of 3 years showed successful function and no cervical bone loss in all patients.
