Sinus Augmentation with rhBMP-2 and Simultaneous Nonsubmerged Implant Placement

Description:
This case features the use of rhBMP-2 and mineralized allograft for grafting the partially pneumatized left maxillary sinus along with simultaneous placement of a large diameter implant in a nonsubmerged mode into the maxillary left second bicuspid site. The step-by-step surgical protocol will focus on incision design, lateral window osteotomy, membrane elevation and repair, the use of rhBMP-2 for bone grafts, and simultaneous non-submerged implant placement. Excellent...as always! Enjoyed this very much. Thank you! Anita

Thank you Mike for this excellent summary of the vital steps in this procedure. Question: I notice a slight buccal concavity from the loss of the premolar some years earlier. Is there any value in buccal augmentation of bone since you are in there or will it just resorb? Thank you.

congratulations for a very interesting combination of procedures. Thanks for sharing

Dr. Pikos. Thank you for your contribution. I was under the impression that we should not mix PRGF/PRF or PRP with BMP-2? That is what I had heard at other lectures and from representatives of Medtronics, Please explain if I misunderstand. many thanks Band

“IPG-DET Technique” The Evolution of New Age Concept
“IPG” DET protocol: a pioneer technique of internal bone regeneration in the sinus without the need of sinus floor elevation (SFE) Patients suffering from maxilla deficiencies are treated with extensive bone transplantation and Sinus Floor Elevation (SFE) procedures, in order to accomplish a successful and stable dental implantation. In 2014, “IPG” DET – called the Ioannis P. Georgakopoulos Dentist Education Institute Technique – has been proven a solid and reliable alternative to SFE. Actually, “IPG” DET is a well-established, simple, safe, efficient and cost-effective dental implantation protocol that combines a complex of fibrin, concentrated growth factors and CD34+ stem cells (fibrin membrane) along with bone grafting and intentional perforation of the Schneider’s membrane towards a rapid implant insertion.

Innovative New Technique for Minimally Invasive Crestal Sinus Lift Approach
This webinar will describe the protocol for a trans-crestal sinus lift by using an innovative product called the Magnetic Mallet. This technique allows the clinician to perform a sinus lift and same time osseodensification with immediate implant placement, in an atraumatic and conservative fashion. Dr Baldi will describe the benefits he has seen in his daily practice as well as the additional opportunities in treating "everyday cases" with this magnetic dynamic technology.

Emerging Concepts in Sinus Augmentation; Atraumatic and Efficient Crestal and Lateral Access Techniques
This presentation will highlight through in depth clinical case documentation, the new and evolving role of instrumentation for access to the Sinus for Bone Augmentation. Efficiency and minimally invasive approaches which increase success and minimize complications will be highlighted. Posterior Maxillary reconstruction often involves Sinus Augmentation and Implants then inserted into the grafted site. A knowledge of useful and clinically manageable techniques that provide access but minimize large surgical flaps and sinus membrane perforations are keys to success.

Sinus Floor Elevation: A Clinical, Radiographic and Histologic Overview
This presentation and imbedded HD videos will highlight the important aspects of the Sinus Lift procedure and how to increase the success rate while minimizing complications. An overview of the procedure with detail radiographic planning and follow up outcomes will be displayed and histoligical results highlighted to gain an appreciation of the timing of bone fill and when you can place and load implants in the augmented sinus. Complications and their management will be equally covered with an impetus on case and patient management and follow up care. Common medications and techniques for each procedure will be noted.
