Description:
Dr. Maurice Salama performs a socket seal procedure in the area around tooth #8. The patient presents inflamed gingiva on the labial aspect due to a vertical fracture. He utilizes GEM 21S from Osteohealth as his socket grafting material, and autogenous connective tissue for the sealing of the graft.

I was under the impression that Bio Oss was to be used in a particulate graft as a "supplement" to hold the shape of the graft site (supplement= sprinkle in perhaps 40% Bio-Oss with the cortico- cancellous allograft). My understanding was that Bio-Oss was so slow to convert to native bone (if it even turns over??) that in 4 or 5 months when you reenter the site you would have Bio-Oss Particles. Ok, I realize I am a bit green but I would appreciate clarification even if clarification is just leading me to another video. Furthermore, I have not seen the rationale for choosing between cortico, cancellous, cortico/cancellous... demineralized vs mineralized... 50/50 or 70/30 mix... I generally understand the notion that mineralized and cortical bone converts at a slower rate but I do not understand what is necessary when and where... thanks I have plenty of time to watch videos!

Shahdad; 2007 PET did not exist yet....but yes!! Today, it would definitely be a PET case. Look how much was done back then to mask the ridge collapse. regards Dr. Salama

Carlos; That makes sense. In the posterior region a collagen plug or PRGF plug would be fine!!

Dr. Salama, what do you think of of using collagen plugs over a preserved site in the posterior region? With ovate pontic on top.... Thanks PS Sometimes we want to minimize the procedures in some patients and avoid touching the palate, thats my concern..

The Dual Zone Approach to Tooth Replacement in the Esthetic Zone
Improvements in implant designs have helped advance successful immediate anterior implant placement into fresh extraction sockets. Clinical techniques described in this presentation will enable practitioners to achieve predictable esthetic success using a method that limits the amount of buccal contour change of the extraction site ridge and potentially enhances the thickness of the peri-implant soft tissues coronal to the implant-abutment interface. This approach involves atraumatic tooth removal without flap elevation, and placing a bone graft into the residual gap around an immediate fresh-socket anterior implant with a screw-retained provisional restoration acting as a prosthetic socket seal device. Long term follow up and evidence from scientific literature to support this concept will be described.
