The Socket Shield Technique - Have the Rules of the Game Changed in Aesthetic Zone Implant Therapy?

Description:
Implant therapy is in the age of being increasingly aesthetically driven, not merely restorative. With the increasing popularity of immediate implants, particularly with anterior tooth extraction, the relevance of socket changes following extraction has come to the fore. Contrary to what was originally believed, the installation of an immediate implant does not arrest bone loss, and immediate implants in the aesthetic zone are prone to recession of bone and soft tissue as a result of bundle bone resorption which is obligatory with tooth extraction. There have been several strategies employed to minimize these aesthetic problems including the dual zone grafting technique, bilaminar connective tissue grafting etc. These require the use of bone grafting materials, additional surgical site morbidity etc. and there still remains an uncertainty of the long term outcomes. The socket shield technique involves leaving behind a sliver of tooth on the facial side attached to the periodontal ligament with the intent of preserving the bundle bone. This presentation reviews some of the problems associated with immediate implant placement, the socket shield technique, and clinical cases to show the technique and its advantages.

Very nice presentation Dr. Nanayan! If the tooth is vital, I assume you make sure you removal all of the pulpal tissue? Thank you.

Excellent presentation Dr Narayan congratulations, I would like to know how are the characteristics of the bur that you use in this technique, maybe do you know if is possible found it in latino america?

Great presentation DrNarayan and few details including the thickness of the sheild for both ant and posteriors wether to graft the GAP and what graft would be ideal and one more imp question is does SS andPET therapies enhance bone to implant contact and does it accelerate or retard healing around implants as compared to conventional immed implants is the question again nice presentation hope to hear from and see you soon

The Potential of Lasers in Advanced Periodontal and Implant Therapy
Periodontal Disease continues to be the single most significant issue in dentistry. It effects the teeth, Implants and overall systemic health of our patients. This presentation describes the utilization of a specific laser energy in the treatment of periodontal bone loss around teeth and implants. The comparison of conventional techniques to laser therapy will also be discussed.

Esthetics Around Implants; Introducing the "Snake" Technique !
The “Snake technique” is a new concept in minimally invasive surgery.  It is based on old fundamental principles that offer reliable and predictable results. The “Snake technique” is a pediculated gingival graft, which unlike free grafts (CTG, FGG) offers several advantages and a lower surgical stress for our patients by having no need for an additional surgical site. As the small details make the biggest differences, the “Snake technique” may add value to both the surgical course of therapies selected as well as the relationship with our patients who demand minimally invasive treatments.

Sequencing of Periodontal Procedures and Orthodontic Treatment Premium Member Content

Sequencing of Periodontal Procedures and Orthodontic Treatment
Severe cases of periodontal disease often require periodontal surgery and realignment of teeth. Surgical techniques have been developed that attempt to minimize postsurgical gingival recession and compromise the interdental papillae. A case report is presented in which reversal and correction of a deteriorating maxillary frontal dentition were effectively achieved through combined use of periodontal and orthodontic principles. The treatment plan included the control of periodontal inflammation, restoration of lost attachment apparatus, realignment of anterior dentition, stabilization of occlusion, and minor periodontal plastic surgery. The anticipated loss of a maxillary lateral incisor was avoided. Restoration of a pleasant smile with nicely aligned teeth and esthetic gingival contours was achieved. The correct sequencing of the procedures involved was considered a key factor for the long-term esthetic outcome.

Emerging Perspectives in Soft Tissue Augmentation: Evolving Tools, Techniques & Materials
Soft tissue root coverage microsurgery and pontic site enhancement are frequently utilized periodontal plastic surgery techniques to create an ideal restorative frame for teeth, pontics and implants. Tunnel, Semilunar, VISTA etc. are among the new age techniques highlighted in this growing area of periodontal microsurgery. New autologous tissue harvest techniques, Allograft ACDM and Bioactive Modifiers have all changed the way we are capable of managing soft tissue deficiencies and have assisted the clinician in improving the healing qualities of these minimally invasive techniques. This lecture will cover site preparation techniques for conventional dentistry as well as prior to and after implant placement.

The Socket-Shield Technique to Support the Buccofacial Tissues at Immediate Implant Placement

The Socket-Shield Technique to Support the Buccofacial Tissues at Immediate Implant Placement
Tooth loss and subsequent ridge collapse continue to burden restorative implant treatment. Careful management of the post-extraction tissues is needed to preserve the alveolar ridge. In-lieu of surgical augmentation to correct a ridge defect, the socket-shield technique offers a promising solution. As the root submergence technique retains the periodontal attachment and maintains the alveolar ridge for pontic site development, this case report demonstrates the hypothesis that retention of a prepared tooth root section as a socket-shield prevents the recession of tissues buccofacial to an immediately placed implant. The socket-shield technique is a highly promising addition to clinical implant dentistry and this case report is among the first to demonstrate the procedure in clinical practice with a 1-year follow up.
