Description:
Dental implant esthetics can be challenging, but these challenges are magnified when a patient has had multiple failed implant and bone grafting surgeries.  This video demonstrates the treatment of one such case.  This is a surgical video demonstrating the VIP-CT (Vascularized Interpositional Connective Tissue) graft.  While multiple treatment options exist, this start-to-finish video highlights the patient's initial presentation, the surgical technique, and the final outcome at 2 years.

Hi Filipe! Thanks so much for the kind remarks. I agree 110% with NOT placing two implants side by side if the goal is to obtain a significant papilla. I much prefer implant-pontic. Unfortunately in this situation, I inherited the patient after the implants had already been placed. And to the surgeon's credit, he inherited a poor situation as well. As far as utilizing ENDORET, I am actually slow to adopt this but am using A-PRF. This was not used in this case, but truthfully at the time, I didn't know a whole lot better. Thank you so much for the perspective, my friend. It was truly a pleasure and honor to meet you as well and I look forward to our next meeting.

Soft tissue recession around teeth and implants is an epidemic problem in the field of dentistry. A predictable and long-term soft tissue augmentation outcome is primarily dependent on proper diagnosis, prognosis and treatment planning. In this lecture and video based Case Report, a patient with multiple recession defects in the Maxillary Anterior Region who was first treated with Tunnel Approach and ACDM grafts reveals breakdown of the result at 3 years in the area of the Maxillary Left Central Incisor with further recession and erosion of the root. The Semi-Lunar Approach is highlighted as a minimally invasive microsurgical approach to this situation. Incision design, instrumentation and suturing are highlighted. Post-op results are displayed.

The Dual Zone Approach to Tooth Replacement in the Esthetic Zone
Improvements in implant designs have helped advance successful immediate anterior implant placement into fresh extraction sockets. Clinical techniques described in this presentation will enable practitioners to achieve predictable esthetic success using a method that limits the amount of buccal contour change of the extraction site ridge and potentially enhances the thickness of the peri-implant soft tissues coronal to the implant-abutment interface. This approach involves atraumatic tooth removal without flap elevation, and placing a bone graft into the residual gap around an immediate fresh-socket anterior implant with a screw-retained provisional restoration acting as a prosthetic socket seal device. Long term follow up and evidence from scientific literature to support this concept will be described.

The loss of alveolar ridge contour in the esthetic zone compromises both esthetics and function. This clinical presentation will focus on the application of both hard and soft tissue grafting in the esthetic zone implementing state of the art interactive CT diagnosis and treatment planning for optimal esthetic implant reconstruction. Emphasis will be on indications, graft and harvest site assessment, timing, and use of bioactive modifiers including BMP, PRGF and PDGF. The surgical protocol utilizing allogeneic and autogenous grafts in conjunction with connective tissue grafts, acellular dermis matrix, and related soft tissue procedures to avoid functional and esthetic pitfalls will be featured. Pre and post-grafting prosthetic workup, interactive CT graft assessment and perio prosthetic driven implant planning, stent driven implant placement, and computer milled abutment fabrication including guided surgery will be included.
