The surgical objectives to meet functional and aesthetic goals include enhancement of both quantity and quality of bone as well as ideal placement of dental implants for restorations that mimic nature. In this presentation, Dr. H. Ryan Kazemi discusses current surgical techniques in implant site development in the posterior maxilla including extraction site grafting, sinus lift, bi-directional bone graft, and osseodensification for bone management.

Thanks Ward and Mo. Great point on class 3 sinus tears- repair certainly possible with thicker membranes, size, and its location.

Thanks for the presentation. Nice cases and nice case management. There is a way to treat Class 3 sinus tears. Dual resorb able collagen membrane technique or even suturing the membrane back together.

Ridge Split Procedure with Simultaneous Implant Placement
A ridge split procedure has been suggested in literature for increasing the bucco-lingual width of the alveolar ridge prior to implant placement. The procedure can be performed in one or two surgical appointments. It is usually combined with additional augmentation utilizing a bone substitute material and a collagen membrane. In this video Dr Kher demonstrates a ridge split procedure that has been facilitated using a piezo surgical device. Following that he places 2 implants for the proposed restoration of a missing maxillary canine and first bicuspid after preparing the ossteotomy site using expansion screws. Thereafter he uses a CPS putty to graft the gap between the implants to fill all voids and achieves a satisfactory tension free closure.

Autologous bone grafting used with dental implants was originally described by Bränemarketal in 1975, and is now a well-accepted procedure in oral and maxillofacial rehabilitation.Placement of an endosseous implant requires sufficient bone volume for complete bone coverage. Furthermore, the pattern of ridge resorption contributes to an unfavorable maxillomandibular relationship, requires angulations of the implants and/or angled abutments, and affects the proximity of adjacent facial concavities(maxillary sinus, nasal cavity) and vital structures(mandibular nerve).
