"The Soft Block": Treating Severe Recessions Around Dental Implants - Part 1 of 2

Severe recessions around implants is a common issue. Unhappily this is a problem we have to face in our everyday practice. To deal with it we have to develop strategies to meet our patients expectations. Recessions and peri-implantitis come together very often. Being capable to improve the biological environment around a dental implant may have huge relevance in it´s long-term prognosis. Treatment algorithms proposed by some authors are often very complicated and sometimes frustrating in what comes to results. Filipe Lopes & Bernardo Mira Correa will try to present simple algorithms and biological ways for trying to solve this kind of clinical challenge, other than implant explantation that must be also, and so many times, considered as the first choice. This is intended to be the first of a series of posts on the subject. We will feature different soft tissue management strategies, including autologous grafts and alloderm. We will underline the importance of: diagnosis,PRGF ENDORET® graft, type of implant, grafting technique to use and allografts that can possibly help us on some challenging situations. Emphasis will be also held on tools, the BTI extraction kit, that allow us an easier removal of implants than ever before, reducing dramatically the biological cost of their loss and making it easier to place a new implant. At the end we will try to provide for a simpler treatment algorithm, that has been of good service for us, for some years now.

Hi Roy! I saw your question just now! Thx for watching. In fact this is a very special type of fully epithelized FGG (free gingival graft), so you don´t cover it. Until the end of october 2018 I will publish a brightbook on the technique.

Thx Mo. I posted 2 years and a half ago and the patient still smiles radiantly. That grey collar showing was really an issue for her. Up until now we were succesfull in avoiding expalnting the implant. See u soon in Vegas!

Surgical Concepts to Treat Esthetic Implant Disaster Cases - Part 2 of 2
The utilization of dental implants in the esthetic zone is a challenge for involved clinicians. With the increasing application of dental implants by non-experienced colleagues, the occurrence of esthetic failures with implant supported restorations is raising. This lecture will address the causes for such esthetic failures and the surgical approach for therapy. In most cases, implant removal is necessary, which must be done with a low-trauma technique to avoid additional bone loss. Today, this is done with special implant removal instruments using the Reverse Torque Technique. In addition, these failures are often characterized by a lack of keratinized mucosa. The current strategies for the reestablishment of keratinized mucosa and for the augmentation of missing bone, mainly on the facial aspects are presented and documented with case reports. This includes aspects of timing, surgical techniques, and the selection of appropriate biomaterials.

Socket Shield Technique is a concept in implant dentistry that utilizes the partial extraction therapy principles, with the goal of preserving the hard and soft tissues around the dental implants. It has been reported to be a very predictable therapy so far. The proper case selection is crucial for the success of the technique. In fact, it is very technique sensitive and requires an advanced level in implant dentistry. Although the technique has a high overall success rate, but long term studies and high evidence level researches are needed to support the proof of principle available.
