Things are indeed evolving. I believe Dr. Pikos would no longer perform this procedure for Sinus Grafting! Today the protocol has evolved to bioengineering using BMP-2, PRGF, PRF or Nothing at all in the Sinus. Let's continue to monitor these protocols on this site and see the long term research of the techniques. Happy New Year Dr. Salama

I agree that it's an intereting video and case. However, we never forget that this procedure could provoke a Weir-Mitchell disease as neuroalgodystrophia (pain and bone loss). I have seen in my pain clinic experience two cases of spontaneous (!) tibia fracture few months after the tibia harvesting for sinus lifting. My opinion is to prefer definity oral bone or biomaterials or nothing (PRF). it's more safe and simple.. Thanks.

When the need for bone grafting is quite extent, experts consider autogenous bone in combination with any other available material on the market is more predictable and secure in terms of bone grafting for wide spaces. Moreover it is expected to provide better stability, avoiding any kind of rejection that might occur by the recipient site due to the extent and other possible causes.
Now, I agree with Henry Salama and Sam Busich that this Tibial Harvest Technique is a nice example of a potential donor site, which can be presented as an alternative to a patient inclined towards having an autogenous bone graft.
It is pretty lamentable that some colleagues cannot appreciate this valuable contribution.
It is a very interesting and educative video.

I repeat what was previously posted by Dr. Henry Salama; "I think the main purpose of the video was to demonstrate another potential donor site for bone harvesting for those preferring autogenous bone alone or in a mixture regardless of whether you use it in the sinus or to augment ridges, sockets etc. If you prefer to use allograft alone in sinuses, Infuse sponges, or even nothing but PRF, that appears to be perfectly legitimate based on the literature". I don't perform the procedure but have worked with surgeons who do when the need for this autogenous bone is required such as COMPLETELY pneumatized Sinus or LARGE dentoalveolar ridge deficiencies or Cleft repairs.

Platelet derived growth factors are now routinely utilized in reconstructive therapy.
