Tissue Engineering and Platelet Derived Growth Factors: Evidence Based Therapy

Description:
Platelet derived growth factors are now routinely utilized in reconstructive therapy. This presentation describes very detailed and evidence based guidelines for clinicians interested in enhancing their abilities in tissue engineering, especially as it relates to bone augmentation. Specifically, while successful bone augmentation requires the standard surgical parameters of space maintenance, low pressure on the grafts and tension-free flap closure, optimization of this goal requires management and enhancement of the local biological conditions with growth factors. Towards that end, platelet concentrates through the constant release of growth factors are able to promote and enhance new vascularization, provide plasma protein, normal lipidemia, as well as increased collagen and fibrin activity.

Great presentation... thank you. You used the prf to cover mucoderm without primary coverage. In other bone grafting situations in which primary closure is a challenge have you successfully seen the graft covered solely by the prf membranes?

Ewa Świech - (6/8/2014 12:07 PM)

thank You doctor for fantastic nd helpfull leture

Gustavo Garces - (6/7/2014 11:48 AM)

Excellent and very informative, Merci bien Dr Choukroun.

I was able to have my pharmacy provide me with the 50 mg Metronidazole without incipients. Most compounding pharmacies should be able to do this.

Maurice Salama - (3/6/2014 7:43 AM)

Ed; Try ACE Surgical Supplies or Medical Suppliers. Dr. Salama

Edward Mazer - (3/5/2014 9:11 PM)

i have tried everywhere to get metronidazole powder or vials for my bone grafting. Where do you get your supply from Ed Mazer

Edison Louie - (1/6/2014 12:10 AM)

Excellent presentation touching on so many pertinent perspectives which truly affect the outcome. Thank you!

Maurice Salama - (10/27/2013 9:17 AM)

Great presentation Joseph. Can you provide us with a preview of your recently released A-PRF protocols? Thanks Maurice

Joseph Choukroun - (7/17/2013 5:01 PM)

Jerry, It's a good indication ! However I prefer to fill the preparation site with the exsudate from the Box.. since few months I launched the Advanced PRF with a specific property: relase of BMP.and 50% of these BMPs are in the exsudate;
You'll be updated for free if you want... best. Joseph

"The Socket Shield Technique"; Myths & Realities
Socket Shield Technique is a concept in implant dentistry that utilizes the partial extraction therapy principles, with the goal of preserving the hard and soft tissues around the dental implants. It has been reported to be a very predictable therapy so far. The proper case selection is crucial for the success of the technique. In fact, it is very technique sensitive and requires an advanced level in implant dentistry. Although the technique has a high overall success rate, but long term studies and high evidence level researches are needed to support the proof of principle available. This presentation will discuss and review the "myths & realities" of the new Socket Shield Technique or "PET" (Partial Extraction Therapy) concept.

Excessive Gingival Display Managed by Surgical Lip Reposition, Aesthetic Crown Lengthening and Veneers
Excessive gingival display can be managed by a variety of treatment modalities, depending on the etiology. Mild and moderate gummy smiles can be successfully treated by periodontal plastic surgery, however for excessive gingival display as in cases of VME II or III with 5 to 8 mm or more of gingival exposure this procedure is not enough. Many patients decline the more invasive Le Fort osteotomy and opt for a less invasive surgical lip repositioning procedure. Currently Botox is used as alternative, but this is only a temporary treatment with very short effectiveness. That is why we suggest surgical lip repositioning as an effective procedure to reduce gingival display by positioning the upper lip in a more coronal location. The long-term stability of the results remains to be seen, but it holds promise as an alternative treatment to more invasive procedures.

The Socket Shield Technique - Have the Rules of the Game Changed in Aesthetic Zone Implant Therapy?
Implant therapy is in the age of being increasingly aesthetically driven, not merely restorative. With the increasing popularity of immediate implants, particularly with anterior tooth extraction, the relevance of socket changes following extraction has come to the fore. Contrary to what was originally believed, the installation of an immediate implant does not arrest bone loss, and immediate implants in the aesthetic zone are prone to recession of bone and soft tissue as a result of bundle bone resorption which is obligatory with tooth extraction.

Ridge Preservation with Modified “Socket-Shield” Technique: A Methodological Case Series
After tooth extraction, the alveolar bone undergoes a remodeling process, which leads to horizontal and vertical bone loss. These resorption processes complicate dental rehabilitation, particularly in connection with implants. Various methods of guided bone regeneration (GBR) have been described to retain the original dimension of the bone after extraction. Most procedures use filler materials and membranes to support the buccal plate and soft tissue, to stabilize the coagulum and to prevent epithelial ingrowth.

The Aesthetic Smile: Diagnosis and Treatment

The Aesthetic Smile: Diagnosis and Treatment
Until recently, dentists' and the public's concept of dental aesthetics was necessarily limited to alterations of the teeth themselves. Dentists concerned themselves with changing the position, the shape and the color of the teeth -basically restoring missing units or enhancing those already present. For the most part the dentist was forced to accept the pre-existing relationship between the three components of the smile; the teeth, the gingival scaffold and the lips.
