Titanium Mesh GBR in Combination with Bioactive Modifiers and Implant Placement

Description:
This Surgical Video will highlight the utilization of a new CTi Titanium Mesh product that intimately connects to the top of the implant to create space for Guided Bone Regeneration. A 3D CBCT is utilized to inspect the recipient site and classify the ridge as a Class 2 Buccal deficiency. Implant placement is then navigated to minimize implant perforation on the buccal aspect and a CTi mesh is then adapted to the top of the placed implant and secured with an appropriate washer and cover screw to maintain the space for Allograft bone and PRGF Fibrin. This approach allows for a easy fixation of the mesh without the need for fixation or tenting screws. This combined approach allows for both the implant and the GBR to take place simultaneously. Incision design and suturing of the flap is also highlighted.

Tareq; See order button to the left of the video to order CTi-mesh. You must order the spacers and caps with the mesh. As for collagen barriers and other particulate materials that would be fine. I prefer PRGF or PRF. Dr. Salama

Good day Dr Salama; When ordering the Ctimesh does it come with a spacer or do use implant manufacturer ones, as well can you use Bioguide as a membrane ontop of the mesh and a mix of Bioss and autogenous bone for grafting?? Tareq

Binh, Gary and Theo; Thanks. You can purchase the product directly to the Left of the Video where it says CTi Membrane BUY button. I believe this to be an efficient technique at space maiteneance specifically around placed implants. Puros should work well as would many other bone graft particles. No advantage to bone quality but as it does NOT resorb it maintains the space indefinitely where resorbable barriers may resorb too quickly.

very nice video and approach for placing an implant with GBR simultanecly. what are the advantages compared to resorb able or non resorb able membranes .(bone quality,quantity or cost) thanks .

looks like a very efficient technique. I will be interested to see how it works, as well as if closure id maintained. How would you anticipate Puros cancellous bone particles to work in this situation when utilizing prp and also the titanium mesh and fibrin?

Thanks Dr. Salama. I've been waiting for this after seeing your previous video last year. Where can I buy this product? The next step should be a similar mesh but bioresorbable (Like Sonic Weld membrane) and can be light cure to hold custom shape (like Triad material). Make it radiopaque too! Is that too much to ask? Advanced Bone Grafting Techniques: Part 1 - Autogenous Bone Blocks in the Reconstruction of the Atrophic Mandibular Ridge
This lecture will describe the use of autologous onlay block bone grafting for reconstruction of moderate to severe atrophic posterior mandibular alveolar ridge. This will include a discussion of the various options to this form of grafting, such as Short implants, Lingual positioning, nerve re-positioning, distraction osteogenesis, and GBR. The Mutli-Tier onlay Bone Graft Technique will be highlighted in this presentation. Additionally, a review of a recently submitted research paper of long term follow up of implants placed into autologous bone blocks harvested from the mandibular ramus, retromolar and sympheseal areas will be discussed. A description of surgical donor harvest sites in the mandible will also be performed as well as post op healing of these sites. Additional time will be spent reviewing causes of failure with bone blocks and implants placed into these sites.
