Crestal bone stability is considered to be important for bone preservation, longevity of implants and to prevent peri-implant tissue recession. Implant placement level is important for bone stability. Mucosal tissue thickness was shown to be the factor having impact on bone stability. It was showed that platform switching does not reduce crestal bone loss, if soft tissues at the implant placement are thin.

thank you doctor,i would like to ask how much vertical soft tissue thickness is considered as thin ?when we should think that this much soft tissue thickness is safe or may be considered as thin or starting thick?

Gerald - I believe that we are at a stage where bone loss should no longer be an expected occurrence. the message is that if the treatment design accounts for the known variables that may influence bone loss, i.e. thin biotype, thin labial bone, biologic remodeling, type of connection, cement, occlusion, hygiene... etc, we can minimize, and in more and more cases, eliminate bone loss.
Tom - I believe that the message is that platform switching protocols alone will not override biological remodeling bone loss in thin tissue environments.
Osama - generally 2mm or more is deemed a minimum thickness of tissue to develop optimum and stable long-term environments.

Bright young man, nice presentation on the advantages of a thick biotype and platform switching, however, he did not address the issue of perimplantitis and regardless of the tissue type....bone loss will occur. Gerald Rudick dds Montreal, Canada

I would imagine that placing an abutment one time and leaving it alone throughout the treatment would also be a factor. Only problem is that you need to use cemented restorations or use some sort of transitional abutment. Very well done. Aciu Labai!

The Socket Shield Technique - Have the Rules of the Game Changed in Aesthetic Zone Implant Therapy?
Implant therapy is in the age of being increasingly aesthetically driven, not merely restorative. With the increasing popularity of immediate implants, particularly with anterior tooth extraction, the relevance of socket changes following extraction has come to the fore. Contrary to what was originally believed, the installation of an immediate implant does not arrest bone loss, and immediate implants in the aesthetic zone are prone to recession of bone and soft tissue as a result of bundle bone resorption which is obligatory with tooth extraction.

It is suggested that thin tissues might be thickened during implant placement, thus reducing bone resorbtion. The role of soft tissue thickness is well established in aesthetic treatment. It is known that thin soft tissues present an unfavorable situation for implant placement, crestal bone stability and subsequent prosthetic treatment.

Platelet derived growth factors are now routinely utilized in reconstructive therapy. This presentation describes very detailed and evidence based guidelines for clinicians interested in enhancing their abilities in tissue engineering, especially as it relates to bone augmentation. Specifically, while successful bone augmentation requires the standard surgical parameters of space maintenance, low pressure on the grafts and tension-free flap closure, optimization of this goal requires management and enhancement of the local biological conditions with growth factors. Towards that end, platelet concentrates through the constant release of growth factors are able to promote and enhance new vascularization, provide plasma protein, normal lipidemia, as well as increased collagen and fibrin activity.

Post-Traumatic Treatment of Maxillary Incisors by Immediate Dentoalveolar Restoration with Long-Term Follow-Up
Replacing both missing maxillary interior teeth is particularly challenging, especially in compromised sockets. The case report describes the management of an 18-year-old female patient, who suffered avulsion of both maxillary central incisors at 7 years of age. The multidisciplinary implant technique, called Immediate Dentoalveolar Restoration (IDR), included extraction of the injured teeth and a single procedure for immediate implant placement and restoration of the compromised sockets after root fracture and peri-apical lesion development were detected during orthodontic treatment. Successful esthetic and functional outcomes and reestablishment of the alveolar process after bone reconstruction were observed during the 3-year follow-up period. The predictable esthetic outcomes and soft and hard tissue stability that can be achieved following IDR are demonstrated.
