The Pontic-Shield: Partial Extraction Therapy for Implant Dentistry
Augmentive ridge preservation techniques aim to manage the postextraction ridge. The partial extraction of teeth may better preserve the ridge form by maintaining the bundle bone-periodontal tissues and preserve the ridge beneath dentures or fixed prostheses. The socket-shield technique entails preparing a tooth root section simultaneous to immediate implant placement and has demonstrated histologic and clinical results contributory to esthetic implant treatment. A retrospective 10-patient case series treating 14 partial extraction sites demonstrates how a modification of the socket-shield technique can successfully develop pontic sites and preserve the ridge.

Ridge Preservation with Modified “Socket-Shield” Technique: A Methodological Case Series
After tooth extraction, the alveolar bone undergoes a remodeling process, which leads to horizontal and vertical bone loss. These resorption processes complicate dental rehabilitation, particularly in connection with implants. Various methods of guided bone regeneration (GBR) have been described to retain the original dimension of the bone after extraction. Most procedures use filler materials and membranes to support the buccal plate and soft tissue, to stabilize the coagulum and to prevent epithelial ingrowth.

The case series evaluated the peri-implant tissue response following extraction and immediate placement and restoration of an implant in conjunction with subepithelial connective tissue graft (SCTG) and bone grafting in the esthetic zone.

Sequencing of Periodontal Procedures and Orthodontic Treatment
Severe cases of periodontal disease often require periodontal surgery and realignment of teeth. Surgical techniques have been developed that attempt to minimize postsurgical gingival recession and compromise the interdental papillae. A case report is presented in which reversal and correction of a deteriorating maxillary frontal dentition were effectively achieved through combined use of periodontal and orthodontic principles. The treatment plan included the control of periodontal inflammation, restoration of lost attachment apparatus, realignment of anterior dentition, stabilization of occlusion, and minor periodontal plastic surgery. The anticipated loss of a maxillary lateral incisor was avoided. Restoration of a pleasant smile with nicely aligned teeth and esthetic gingival contours was achieved. The correct sequencing of the procedures involved was considered a key factor for the long-term esthetic outcome.

This course will provide the latest evidence-based information on peri-implant diseases with emphasis on a team/interdisciplinary approach to treatment. The program will present the updates on the definition and etiology of peri-implantitis from the American Academy of Periodontology’s 2018 consensus meeting. The instructor will present research findings on peri-implant disease diagnosis and management.

Immediate implant placement in the aesthetic zone has become a controversial topic with regards to the long term aesthetic demands that both the profession and our patients have placed on this treatment modality. Since the controversy broke a few years ago there have been a number of outstanding developments that have revolutionized the way we do immediate implant placement. Soft tissue augmentations using the tunnel technique as well as the socket whiled technique are 2 of these techniques which form the baseline of treatment in our clinic. More importantly it will cover numerous complications that we have had to deal with over the last few years that we have been doing this technique.

This presentation will focus on the interdisciplinary relationship of the restorative dentist, periodontist and orthodontist to reconstruct the soft tissue foundation for all of these restorative options in anterior tooth replacement. The diagnosis of deficiencies as well as the varied treatment options will be discussed in detail. This includes periodontal crown lengthening, esthetic periodontal plastic soft tissue grafting procedures as well as prescription adjunctive orthodontic tooth movement to manipulate the soft tissue foundation prior to or subsequent with the restorative options of implants, bridges, or pontic replacement.
