Cortical Tenting Grafting Technique in the Severely Atrophic Alveolar Ridge for Implant Site Preparation

Summary:
Alveolar ridge augmentation using autogenous block grafts is a predictable method to augment localized alveolar ridge defects for implant placement. Large atrophic
edentulous segments present a more challenging scenario in reconstruction,because there is a limited supply of
intraoral donor bone. Extraoral donor sites are an option, but present an obstacle to patient treatment acceptability because of increased costs and morbidity. The purpose of this case study was to evaluate the effectiveness of using particulate human mineralized allograft in combination with an intraoral cortical block graft, in a “tenting” fashion, to augment large intraoral alveolar ridge defects for implant placement.

Crestal Bone Stability around Implants with Horizontally Matching Connection after Soft Tissue Thickening: A Prospective Clinical Trial
It has been shown that thin mucosal tissues may be an important factor in crestal bone loss etiology. Thus,
it is possible that mucosal tissue thickening with allogenic membrane might reduce crestal bone loss.
Purpose: The purpose of this study was to evaluate how implants with traditional connection maintain crestal bone level
after soft tissue thickening with allogenic membrane.
After 1-year follow-up, implants in group A had 1.65 1 0.08-mm bone loss mesially and 1.81 1 0.06 mm distally.
Group B had 0.31 1 0.05 mm mesially and 0.34 1 0.05 mm distally. C group implants experienced bone loss of 0.44 1
0.06 mm mesially and 0.47 1 0.07 mm distally. Differences between A and B, and A and C were significant both
mesially and distally, whereas differences between B and C were not significant mesially and distally

A New & Improved Approach to Zygomatic Implants
Over the past decade numerous approaches to the rehabilitation of severe maxillary atrophy have been suggested.
Most approaches involve the utilization of the Zygoma for anchorage of specially developed extra long Implants.
These implants often must either enter through the sinus or pass along the outside of the sinus wall depending upon the transverse discrepancies and horizontal ridge resorption issues as well as future occlusal scheme with the mandibular arch.
This new approach described in this presentation addresses this potential compromise through extra-maxillary bone grafting performed at the time of Zygoma placement utilizing
fresh frozen iliac bone and bone marrow aspirate. Multiple cases will be highlighted and short term follow up described.

Digital Workflow for Anterior Implant Aesthetics
We have seen excellent results in the aesthetic zone when immediate implant placement techniques have been used. A review of the current literature reveals and confirms the re-education of mid-buccal facial recession is best when an immediate provisional is used. If we can digitize this information and create a crown-root form matrix, we can allow tissue to maintain the exact 3D position of the crown and root of the tooth that is to be replaced. A technique using "dicom" files and "STL" files of the original tooth-root will be demonstrated. This will allow ideal healing of the surrounding hard and soft tissues in for any implants restoration.

The Role of Digital Technologies & Materials for Full-Arch Implant Reconstruction
In prosthetic rehabilitations, we used for many years codified and widely described protocols in literature, with results that met our expectations and well above those of our patients.
