Current Concepts in Bioactivity and Regeneration
An injury to the tissue instantly stimulates the healing process. To heal properly, we need three elements: scaffolds, cells and growth factors. If one of these three elements are missing there will be no tissue regeneration. Plasma rich in growth factors (Endoret® (PRGF®) technology, enables clinicians to apply autologous growth factors and a fibrin scaffold in a simple, safe and effective method. The Endoret® (PRGF®) stimulates the tissue-building cells to achieve regeneration. An optimal alveolar regeneration is characterized by fast haemostasis, adequate inflammation, stem cell mobilization and differentiation, rapid soft tissue closure of the socket and correct tissue regeneration. The adequate management of the extraction socket will determine the final outcome of implant-supported restorations.

Fibrin-Guided Bone Regeneration: Guidelines of a Simple, Predictable, & Low Risk Technique
This lecture is focused on an effective regenerative approach, but at low knowledge. The goal is to provide a new skill in favor of well-established techniques. The Fibrin Sealant (FS) is an additional clot of human origin. It has adhesive, sealant and inductive action stimulating hard and soft-tissue regeneration. The FS, mixed with bone chips and biomaterials, allows a better handling of the graft that, becoming plastic and moldable, adapts perfectly to bony defects without dispersion of granules. It’s effective also on wound protection achieving a faster healing of soft-tissue. This approach is excellent in the socket preservation and in the peri-implant regeneration on "closed" sites where the implants are inside the defects.

The “classical” subperiosteal implant does not enjoy a good reputation; the causes of failure were myriad, as evidenced in the literature. When considering the future of implants, it appears justified to reconsider the old concepts in light of the availability of contemporary technologies, such as computer-aided design, softwares and programs design, virtual stress– strain testing, and 3D printing of titanium alloy. With the employment of these modern technologies, the concept of a “high-tech” subperiosteal implant has gradually emerged. The Khoury Bone Plate Technique is a predictable surgical method to graft alveolar ridge defects in both the horizontal and vertical dimensions. This technique involves harvesting cortical plates from the ramus of the mandible and fixating these bone plates to the ridge defect using screws. Scrapings of cancellous and cortical autogenous bone are then interposed between the plates in a “Sandwich” technique allowing for a 3-dimensional augmentation of the ridge defect.

This presentation analyzes the interpositional bone graft or inlay technique, discussing the reconstructive surgical approach. Horizontal osteotomy with the interposition of bone in the form of a “sandwich” involves raising a coronal osteotomized segment of the mandible, which is still attached to the lingual periosteum, and interpositioning a block bone graft. This technique guarantees a dual vascular supply to the inlay graft from the lingual periosteum and from the residual bone; it also allows optimum use of the native basal bone, which should be less prone to resorption. The advantages and disadvantages of the inlay technique are compared with other commonly used augmentation techniques in the management of posterior mandibular atrophies.

If a tissue want to live long, it has to follow 2 conditions: The first condition is to organize a full blood supply.. However, it’s not enough. The solution for the long term stability is to try to organize the stability of the blood supply.. by the respect of several biologic conditions. Almost of these conditions are explained in this lecture. We introduce here the new concept to avoid the reduction of blood supply by the periosteal incision: the soft brushing technique is the first technique which allows a very large increase of the flap without any incision: the flap closure without tension but without any incision.

In this case that I originally presented at the dental XP summit last August at NYU but did not show the final results, we pretty much used the entire armamentarium currently available to us dentists. Many different procedures and protocols were applied in this complex case. The patient's main concern was long-lasting natural results. We decided to take no shortcuts and to practice as many gold standard procedures as possible, and thankfully the patient could afford the treatment. We are very happy with the outcome, might not be perfect but she is very happy. It has been six months now since we finished and everything is quite stable, and according to what we have been seeing here on XP and in the literature should remain quite stable for a long time.

The utilization of dental implants in the esthetic zone is a challenge for involved clinicians. With the increasing application of dental implants by non-experienced colleagues, the occurrence of esthetic failures with implant supported restorations is raising. This lecture will address the causes for such esthetic failures and the surgical approach for therapy. In most cases, implant removal is necessary, which must be done with a low-trauma technique to avoid additional bone loss. Today, this is done with special implant removal instruments using the Reverse Torque Technique. In addition, these failures are often characterized by a lack of keratinized mucosa. The current strategies for the reestablishment of keratinized mucosa and for the augmentation of missing bone, mainly on the facial aspects are presented and documented with case reports. This includes aspects of timing, surgical techniques, and the selection of appropriate biomaterials.

Treatment of the extremely atrophied edentulous maxilla is one of the most challenging problems in reconstructive surgery. Due to the size of the graft and inherent co-morbidities of these patients they some of the most difficult and challenging to treat requiring multiple surgeries with significant morbidity. The gold standard of care has long been the use of autologous block grafts from the iliac crest prior to implant placement however harvesting bone from the iliac crest comes with significant donor site morbidity. New treatment modalities using minimal invasive procedures and the use of Bone marrow stem cells and growth factors will be discussed.

Numerous rules of tissue engineering have to be applied to maintain the regenerated bone through an adequate blood supply: this lecture is an enlightenment on the biological and mechanical conditions for long term stability of the bone: “grafted bone” or “bone around implants”.

Ridge Split Procedure with Simultaneous Implant Placement
A ridge split procedure has been suggested in literature for increasing the bucco-lingual width of the alveolar ridge prior to implant placement. The procedure can be performed in one or two surgical appointments. It is usually combined with additional augmentation utilizing a bone substitute material and a collagen membrane. In this video Dr Kher demonstrates a ridge split procedure that has been facilitated using a piezo surgical device. Following that he places 2 implants for the proposed restoration of a missing maxillary canine and first bicuspid after preparing the ossteotomy site using expansion screws. Thereafter he uses a CPS putty to graft the gap between the implants to fill all voids and achieves a satisfactory tension free closure.

A Clinical Update in Bone Regeneration: Autogenous Bone vs. BMP-2
For historical and biological reasons autogenous bone has long been considered the gold standard of bone graft materials. For bone augmentation procedures autogenous grafts provided predictable volume gains, favorable bone quality with short healing periods. However, the need to harvest the graft and associated morbidity are inherent disadvantages. The choice of a technique for bone augmentation is dependent on a number of factors including the use of bone substitutes. More recently recombinant human bone morphogenetic protein (rhBMP-2) has been shown to be effective in repairing buccal wall defects from extractions and in sinus bone grafting. The use of rhBMP-2 is currently under clinical investigation as a replacement for autograft in ridge augmentation process.

The defect is treated by utilizing a mix of autogenous bone harvested using Auto-bone collector and cortical-cancellous particulate Allograft. The mix is then secured by a customized 3D pre-formed titanium membrane followed by "labial contour" augmentation of the defect which is then secured by a rigid absorbable collagen membrane to create and maintain space. This will protect the graft and help stabilize the blood clot during the critical healing time. This membrane is fixed by bone tacks for stability. The final results showing good healing of the soft and hard tissues is displayed.

Sonic Weld and BMP-2 for Bone Augmentation of a Severe Anterior Maxillary Defect
This video highlights the diagnosis and treatment of a Severe Type 3 Vertical deficiency in the Anterior Maxilla. The utilization of a new resorbable rigid Poly Lactic Acid (PLA) membrane and the technique of Sonic Weld is highlighted using clinical video and animation. The process of debridement of the site and then shaping and securing the rigid PLA pins and mesh are featured utilizing clinical video and animation. The additional use of BMP-2/ACS and Mineross in a 50-50 mixture is used a bone graft substitute. Tension Free closure, suturing and early healing are also displayed.
