Combination Therapy for Soft Tissue Grafting Procedures

Description:
Realization of predictable outcomes with shape, shade and unique characteristics of dental restorations has proven to be attainable and maintainable due to the static nature of a carefully prepared foundation and the overlying superstructures. On the other hand, the supporting soft tissue frame, which in turn is controlled by the three-dimensional anatomical characteristics of its surrounding environment, is further complicated by an inherent dynamic nature that is also dependent on a host of biological factors for long-term maintenance of ideal surgical outcomes. Realities and facts vs. hype will be discussed in this presentation based on a review of clinical results achieved and maintained utilizing combined soft tissue grafting approaches with up to 22 years of post-operative follow-up.

The treatment of periodontally compromised patients has been significantly altered by the availability of predictable implant dentistry.Where in the past, a compromised tooth with advanced bone loss and/or furcation invasion might have been retained, the high success rate of osseointegrated implants challenges the clinical decision. Should the tooth be saved with periodontal regeneration or be replaced with an implant? Conversely, new biomimetic mediators have enhanced the predictability of regenerative outcomes and encourage endeavors to maintain the natural dentition. This lecture will address the clinical decision that occurs in every practice: save the tooth or place an implant.

Platelet derived growth factors are now routinely utilized in reconstructive therapy. This presentation describes very detailed and evidence based guidelines for clinicians interested in enhancing their abilities in tissue engineering, especially as it relates to bone augmentation. Implant therapy is in the age of being increasingly aesthetically driven, not merely restorative. With the increasing popularity of immediate implants, particularly with anterior tooth extraction, the relevance of socket changes following extraction has come to the fore. Contrary to what was originally believed, the installation of an immediate implant does not arrest bone loss, and immediate implants in the aesthetic zone are prone to recession of bone and soft tissue as a result of bundle bone resorption which is obligatory with tooth extraction.
