Mastering Anterior Composites: 3D Layering Technique

Description:
Traumatic dental injury especially in childhood is a common problem. It occurs usually around 8-12 years old, in a male to female ratio of 2:1 / 3: 1 and affects 70% maxillary central incisors. Coronal fracture of enamel and dentin without pulp involvement is the frequent situation a restorative dentist has to manage, utilizing direct adhesive procedures. Successful shade matching in composite restorations is considered as one of the most difficult task to accomplish. It is based on a subjective comparison between a shade tab and the natural tooth and as direct composite restorations are realized within one single appointment, no "try-in" is possible. Therefore the search for a reliable shade matching procedure coupled with a predictable layering technique is still going on. The aim of this presentation is to present an individual three-dimensional layering technique with contemporary composites. This novel procedure is indicated in moderate to extensive crown fractures and is considered predictable after eliminating the parameter of thickness while layering restorative materials.

Digital Workflow for Anterior Implant Aesthetics
We have seen excellent results in the aesthetic zone when immediate implant placement techniques have been used. A review of the current literature reveals and confirms the re-education of mid-buccal facial recession is best when an immediate provisional is used. If we can digitize this information and create a crown-root form matrix, we can allow tissue to maintain the exact 3D position of the crown and root of the tooth that is to be replaced. A technique using "dicom" files and "STL" files of the original tooth-root will be demonstrated. This will allow ideal healing of the surrounding hard and soft tissues in for any implants restoration.

Introducing Choukroun’s Platelet Rich Fibrin (PRF) to the Reconstructive Surgery Milieu
Platelet-rich fibrin (PRF), developed in France by Choukroun et al (2001), is a second generation platelet concentrate widely used to accelerate soft and hard tissue healing. Its advantages over the better known platelet-rich plasma (PRP) include ease of preparation/application, minimal expense, and lack of biochemical modification (no bovine thrombin or anticoagulant is required). PRF is a strictly autologous fibrin matrix containing a large quantity of platelet and leukocyte cytokines. This article serves as an introduction to the PRF “concept” and its potential clinical applications.
