Thanks gentlemen for the kind response. Glad you enjoyed the lecture. Spencer what I mean by having no buccal bone in a premolar is that even with no buccal bone one gets good healing fo the ridge and an implant can be placed in the correct 3D position without any bone graft being done. So don't do socket preservation and don't put anything in the socket jut let it heal with a blood clot. I hope this helps.

Thanks for posting this lecture -- an awesome overview of bone grafting techniques. I just have one question - around 12:15 you mention "if you have no buccal plate on a premolar, you can extract the tooth, wait 3 months, and you don't need to do grafting." I'm not sure what you mean by this -- if the buccal plate is missing, wouldn't grafting be necessary?

Current Concepts for the Preservation of the Post-Extraction Alveolar Ridge
Different techniques have been presented in literature in order to preserve the post-extraction site; including the use of barrier membranes, tri-dimensional matrix and bone fillers. An “open-healing” approach is nowadays commonly used, combining a proper management of soft tissues with biological integration of biomaterials. Once hard tissue has been successfully augmented, an adequate soft tissue thickness becomes mandatory in order to create healthy and stable peri-implant tissues. An innovative volume-stable collagen matrix provides the opportunity to treat soft tissue defects around implants as an alternative to connective tissue graft.

Retrospective Evaluation of Crestal Bone Changes Around Implants With Reduced Abutment Diameter Placed Non-Submerged and at Subcrestal Positions: The Effect of Bone Grafting at Implant Placement
One method to measure the success of dental implant treatment is to evaluate marginal peri-implant bone-level changes and stability over time. The location of the fixture–abutment interface (FAI) can be of major importance when the goal is to construct esthetic restorations. In these situations the FAI is often placed in a more apical position to create an ideal emergence profile for the prosthetic construction. However, several animal studies have reported that placement of the FAI in a subcrestal position may result in peri-implant marginal bone loss. The aim of the present study is to evaluate the effect of bone grafting of the defect between the bone crest and the coronal aspect of the implant for implants with reduced abutment diameters placed non-submerged and in subcrestal positions.
