David; Good question. I am not sure why not. As for today, I would most likely use a similar approach but with a Tunnel Approach and PRGF/PRF instead. Thanks Dr. Salama

Dear Dr.Salama. Thank you for the great presentation. I got three questions for you. Is the complete submergence of the graft also necessary for palatal connective tissue grafts and if so - where is the root coverage effect beside the thickening of the tissue? Why do you generally prefer connective tissue grafts? Is there a greater risk of inflammation with allografts? Do you therefore always have to use PRGF? Don´t you risk the papilla with these flap designs. I wonder why the tunnel technique is not utilized in the other videos about alloderm as well - like Dr. Allen´s presentation. Best regards.

No difference in the way you suggest suturing and I prefer to trim the alloderm and any sharp edges. Most important not to have alloderm over vertical incision lines.

Hi Dr. Salama, thank you for the great video. I have two questions for you: 1. Would it make any difference if you were to secure the Alloderm individually first using something like 5-0 Vicryl first then coronally advance the flap to completely cover it ? 2. Is trimming the sharp edges of the Alloderm advantageous in any way or not ? Once again thank you and looking forward to your reply.

Improvements in implant designs have helped advance successful immediate anterior implant placement into fresh extraction sockets. Clinical techniques described in this presentation will enable practitioners to achieve predictable esthetic success using a method that limits the amount of buccal contour change of the extraction site ridge and potentially enhances the thickness of the peri-implant soft tissues coronal to the implant-abutment interface. This approach involves atraumatic tooth removal without flap elevation, and placing a bone graft into the residual gap around an immediate fresh-socket anterior implant with a screw-retained provisional restoration acting as a prosthetic socket seal device. Long term follow up and evidence from scientific literature to support this concept will be described.

The Use of PerioDerm for Root Coverage and Correction of Insufficiently Attached Gingiva
The objective of this article is to demonstrate the benefits of an acellular dermal matrix called PerioDerm™ Acellular Dermis in correcting recession and adding connective tissue during periodontal-restorative comprehensive treatment. 1,2 It has long been established that connective tissue is an integral factor in the protection of the underlying periodontal foundation. Connective tissue tends to be tenacious in makeup and have far less vascularization than mucosa. It prevents bacterial infiltration to the underlying supportive periodontal tissues, and in restorative applications, it adds stability to gingival areas during impression taking and cementation of restorations. Though the quality of tissue is critical, it is also important that the tissue is bound down to either tooth surface or bone to serve as a supportive mechanism for protection of the periodontium.

Recombinant Human Platelet– Derived Growth Factor: A Systematic Review of Clinical Findings in Oral Regenerative Procedures
The use of recombinant human plateletderived growth factor–BB (rhPDGF) has received Food and Drug Administration approval for the treatment of periodontal and orthopedic bone defects and dermal wound healing. Many studies have investigated its regenerative potential in a variety of other oral clinical indications. The aim of this systematic review was to assess the efficacy, safety, and clinical benefit of recombinant human plateletderived growth factor (rhPDGF) use for alveolar bone and/or soft tissue regeneration. Based on the clinical evidence, rhPDGF is safe and provides clinical benefits when used in combination with bone allografts, xenograft, or β-TCP for the treatment of intrabony and furcation periodontal defects and gingival recession or when used with allografts or xenograft for GBR and ARP.
